POSITION SUMMARY
Reporting to the Client Services Manager, the Care Team Lead oversees safe, efficient delivery of community support services, including staff leadership, client care coordination, and quality/risk management. This role directly supervises PSW Attendants, leads care planning and assessments, and ensures exceptional client-centered service in alignment with regulatory and organizational standards.
SPECIFIC ACCOUNTABILITIES
Client & Care Management
- Leads intake, assessment, service planning, goal planning (incl. ACP), and safe discharge.
- Conducts annual RAI-CHA and home risk assessments; maintains biannual reassessments as well as ongoing changes to interventions and care plans.
- Facilitates weekly client/attendant support visits; monitors service quality and changing needs.
- Manages annual service agreements and care documentation.
- Supports system navigation and quality aging-in-place outcomes.
Team Leadership & Labour Relations
- Facilitates quarterly Attendant team meetings and monthly/bi-monthly 1:1s (community & head office).
- Monitors dashboards & performance indicators; leads performance conversations and coaching.
- Participates and leads union meetings, grievances, and performance case resolution. Maintains knowledge of collective agreements.
- Supports RTW / modified work planning with HR and supports the health and safety practices of the workplaces in different environments (office, community, car..)
- Recruits, orients, evaluates, and mentors Attendants and ensures compliance with policy/training.
Quality, Risk & Operations
- Audits documentation, monitors service standards, and contributes to QIP and workplan initiatives.
- Regularly updates and maintains care plans of all clients and consumers.
- Investigates incidents and safety events; mitigates risk and ensures safe work practices.
- Participates in meetings, operational reviews and program improvement discussions.
- Participates in on-call rotation and escalates critical client/staff issues appropriately.
- Prepares reports/statistical data as required.
QUALIFICATIONS
- Post-secondary education in health or social services; 3–5 years supervisory experience (unionized healthcare preferred).
- Strong knowledge of PSW scope of practice and community care models.
- Experience with care planning, case management, and InterRAI-CHA (asset).
- Advanced scheduling / AlayaCare experience.
- Strong communication, coaching, digital literacy, and ability to manage sensitive situations.
- Valid driver's license and reliable vehicle.
WORK ENVIRONMENT

§ Sitting, standing, walking, moderate to heavy lifting up to 50 lbs
