Role Summary:
The Clinical Leader is accountable for overseeing the care of a group of patients at the unit (microsystem) level and drives excellence in patient safety and quality outcomes. This position supports the Excellent Care for All Act which places the patient in the centre with attention to care that is of high quality, evidence based and cost effective. The role provides mentorship and education at the bedside and role models teamwork, communication, autonomous clinical practice, critical thinking, best clinical practices, quality care and patient safety standards.
Key Qualifications, Skills and Abilities:
- Current certification with the College of Nurses of Ontario is required
- Bachelor of Science in Nursing or equivalent experience paired with evidence of degree in progress (or commitment to commence the degree process in the near future)
- Masters degree is an asset
- Previous experience in Geriatrics Nursing an asset
- Current ACLS, PALS certification an asset
- Experience using MEDITECH, SCM, ChartMaxx, MedWorxx, MS Office
- Minimum five years clinical experience with a demonstrated understanding of technical and/or clinical information related to the designated Health System noted above.
- Previous experience in a formal nursing leadership role is preferred
- Experience working with an inter-professional team
- Experience as a patient advocate is a definite asset
- Ability to coordinate and delegate duties
- Ability to coach, mentor and provide feedback while fostering a positive work culture
- Excellent communication and facilitation skills – both oral and written
- Excellent interpersonal skills and the ability to work collaboratively with all members of the health care team
- Clinical and leadership skills, which develop and empower members of the health care team in autonomous decision-making to achieve desired outcomes
- Ability to provide leadership to unit based projects and accomplish established goals
- Strong critical thinking, decision making, problem solving and conflict resolution skills
- Commitment to evidence-based practice and quality improvement
- Commitment to patient-centered care including process and outcome evaluation
- Working knowledge of change process/theory and the ability to manage change
- Satisfactory performance and attendance required
Accountabilities and Responsibilities:
- Provide leadership and mentorship to front line interdisciplinary team members to ensure comprehensive and integrated care planning occurs for patient to achieve outcomes that are of high quality and safe
- Oversee the integration of inter-professional care for a designated group of patients (within a unit / microsystem) and adapts based on changing conditions and priorities
- Coordinate, manages, organizes and prioritizes day-to-day patient care activities, staffing and resource utilization for the in-patient unit cluster
- Serve as a direct care provider for complex situations, as appropriate
- Assess, coordinates and evaluates the plan of care with patients, families and interdisciplinary team for the designated group of patients, which can include management of systems and side effects of treatments, coping strategies and monitors response to treatment
- Monitor patient's progress against plan of care until discharge from designated patient care area
- Act as the primary liaison for physicians, interdisciplinary professional team, families and community resources
- Lead team meetings and rounds with focus on outcomes related to quality and provision of safe care
- Support a patient service delivery system that fosters professional autonomy and accountability and patient-centred care
- Monitor and addresses patient and family satisfaction with care, health and safety issues along with the physical environment for patient care to meet our standards for best practices
- Assess, evaluates and anticipates risk management initiatives and ensures patient and team safety
- Promote development of clinical practice improvements and implementing evidence-based best practice that improve outcomes
- Provide patient/family clinical education using available tools and resources
- Assume responsibility for educating, mentoring, guiding and evaluating the health care professional team as appropriate
- Create learning opportunities and uses appropriate teaching/learning strategies, materials and technology to facilitate the learning of the inter-professional team at the bedside
- Participate in hiring, orientation and performance evaluation
- Lead/facilitate unit based teams to achieve unit goals & objectives
- Participate in the Health System and hospital-wide initiatives that support the strategic direction of the organization
- Utilize new technology and evaluates its impact on staff, patients, families and overall best practices
