Vancouver Coastal Health is looking for a Rehabilitation Assistant to join the Home Health team supporting Robert and Lily Lee Family and Pender Community Health Centres in Vancouver, BC. Apply today to join our team

As a Rehabilitation Assistant with VCH you will make your impact by:

• Function under the operational supervision of the Manager or Team Leader and the work direction of a licensed designated occupational therapist or physiotherapist to work as a member of the health care team.
• Provide assigned physical / occupational therapy therapeutic interventions.
• Assist in modification of OT/ PT care plans and facilitate transition to community programs in order to enable clients to maximize potential for independence.
• Provide functional direction to family, report relevant observations of client's condition in writing or by utilizing basic computer literacy.
• Manage equipment safety and inventory.

ABOUT HOME HEALTH PROGRAMS:

Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.

THE CLIENT POPULATION:

Home Health clients typically have on-going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. Robert and Lily Lee Family serves the Grandview, Woodlands and Commercial Drive neighbourhoods, and Pender serves Gastown, Chinatown, Strathcona and the Downtown Eastside neighbourhoods.

Qualifications:

Education & Experience

• Completion of a recognized Diploma level Rehabilitation Assistant Program with Occupational and Physical therapy components plus one (1) year's recent, related experience working with target population.
• Valid BC Drivers License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.
