Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our growing Home Health Care team, serving patients in the Downriver and Washtenaw County areas.
As an LPN, you will provide skilled nursing care, monitor patient progress, and ensure continuity of care in coordination with the Registered Nurse Case Manager. This is a part-time, per-visit position ideal for nurses who want flexibility while making a meaningful impact in their community.

Responsibilities

Deliver high-quality nursing care under the supervision of the RN Case Manager

Monitor vital signs, wound healing, and symptom changes, reporting updates promptly

Administer medications and perform prescribed nursing procedures
-
Document visits, interventions, and patient progress accurately in EMR
-
Educate patients and caregivers on home safety and disease management
-
Coordinate with the clinical team to ensure continuity of care

Promote comfort, dignity, and independence for each patient served
Qualifications

Active Licensed Practical Nurse (LPN) license in the State of Michigan

Minimum one year of experience in home health care preferred

Strong communication, observation, and documentation skills
-
Reliable transportation and a valid driver's license
-
Compassionate and professional approach to patient care
