About the Humber River Family Health Team
The Humber River Family Health Team (HRFHT) is a multi-site, interprofessional primary care organization serving thousands of patients across Toronto's northwest corridor. Our team includes family physicians, nurse practitioners, registered nurses, dietitians, social workers, pharmacists, chiropodists, and other allied health professionals who work collaboratively to deliver accessible, patient-centred, and equitable care.
We are committed to advancing health equity, system integration, and innovative models of community-based primary care that respond to the evolving needs of our patients and communities.
The Opportunity
The HRFHT is seeking dedicated, community-minded leaders to join our Board of Directors. Board members provide strategic oversight, fiduciary responsibility, and governance leadership to ensure the organization achieves its mission, vision, and strategic goals.
This is an exciting opportunity to help guide one of Toronto's most diverse and innovative primary care organizations as it continues to strengthen community health and system partnerships.
Preferred Skills and Experience
We welcome applications from individuals with expertise in one or more of the following areas:
- Healthcare administration, primary care, or clinical practice
- Finance, audit, or risk management
- Governance and organizational strategy
- Human resources, labour relations, or change management
- Equity, diversity, and inclusion (EDI)
- Community engagement and partnership development
- Legal, policy, or public sector leadership
- Communications, marketing, or public relations
Eligibility
- Must be at least 18 years of age
- Must not be a current employee or contractor of HRFHT
- Must reside, work, or have a demonstrated connection to the Humber River community or the broader Northwest Toronto region
Commitment
- Attend approximately 6–8 Board meetings per year (typically weekday evenings, virtual format)
- Participate in at least one standing committee (e.g., Finance, Governance, Human Resources, or Program Committee)
- Serve a two-year renewable term
