Today, the Equity Secretariat has become the Equity, Anti-Racism, and Inclusion Office, with a specific focus on dismantling anti-Black racism and white supremacy and advancing other dimensions of equity within THP. This office supported the completion of an externally led Anti-Black Racism Climate Review and resulting report, which includes recommendations and actions that are being integrated into THP's operating plan. At IBH, the Anti-Black Racism Allyship Initiative (ABRAI) was established in 2020 to understand the experiences of Black staff within IBH and develop an action plan in partnership with Black staff to address anti-Black racism. The action plan has been translated into operational objectives that support the recruitment, retention, and career development of Black staff. As part of this plan, IBH has established an optional, monthly Black Excellence meeting for Black staff at IBH where the purpose is to provide a safe space where all hold power and attendees can connect, network, and support each other. 

The Role 

We are looking for an experienced professional to fill the key role of Clinical Trial Nurse for virtual prostate cancer survivorship research under Dr. Andrew Feifer.

The successful candidate will be working at Trillium Health Partners, as a part of the Ned (No Evidence of Disease) Nurse Clinic. The Ned Nurse Clinic aims to design and evaluate a virtual nurse-led model of care facilitated by telemonitoring to support prostate cancer patients in the survivorship phase of their cancer trajectory. As a member of the Ned Nurse Clinic team, the Nurse engages in managing the remote patient monitoring and the related provision of nursing services for the management of prostate cancer survivors enrolled in the research program. Specifically, the Ned Nurse Clinic seeks to: (1) improve the patient care and survivorship experience; (2) optimize both clinical and self-management of prostate cancer survivorship care; and (3) address the spectrum of clinical and practical patient needs through the delivery of whole person healthcare. The Nurse will act within their full scope of practice to provide clinical care that includes, but is not limited to: 

Key Responsibilities:

Clinical Practice and Research

  • Supports with patient enrollment in the Ned Nurse Clinic and providing program education and training related to Ned application use and remote monitoring program services
  • Provides direct remote care to a defined population of prostate cancer survivors enrolled within the Ned Nurse clinic 
  • Actively monitors the Ned system to review and manage patient alerts. A part of this management will include nursing intervention within the full scope of practice (e.g. symptom management, health education and promotion, managing patients with complex chronic conditions, etc.) and/or escalating care where necessary to appropriate members of the patient's care team (i.e. Specialists within the appropriate institution)
  • Monitors, evaluates and revises the plan of care and therapeutic interventions based on current evidence and on patient goals, preferences, health status, and outcomes
  • Conducts virtual check-in visits with patients as needed for their follow-up care delivery 
  • Supports the implementation of health promoting and self-management support interventions, including the delivery and evaluation of self-management support resources 
  • Provides patients and caregivers with pro-active self-management support interventions that are tailored to meet the individual needs of oncology patients and their families 
  • Acts as the central point of contact for patients and their caregivers and responding to their health concerns. This will include monitoring the Ned asynchronous message feature to respond to ad-hoc patient inquiries 
  • Maintains appropriate clinical documentation of patient care in the institutional medical record 
  • Identifies, collects data on, and evaluates the outcomes of, Registered Nurse practice, for patients and the health-care system
  • Engages with the research team and participates in program evaluation of the effectiveness of nursing interventions 

Interprofessional Collaboration/Consultation/Referral 

  • Coordinates communication between relevant members of the interdisciplinary care team as required for patient care (i.e. Primary Care Providers, Institutional Care Team (Nurse Practitioners, Physicians, Allied Health)) 
  • Provides updates to relevant members of the patient care team as required for the continuity of patient care. This may include regular interdisciplinary communication touchpoints and ad-hoc communication touchpoints related to changes to the patient's health status and/or care plan 

Leadership

  • Provides leadership in the development and integration of the Clinical Trial Nurse role within the health-care system and the virtual care landscape
  • Examines current processes, identifies opportunities for change, and provides recommendations for improvements
  • Provides leadership in the management of clinical research care and virtual care innovation 

Qualifications

The following lists required competencies for the Clinical Trial Nurse role:

  • Graduate degree from an approved and accredited program
  • Current registration with the College of Nurses of Ontario – Registered Nurse with no restrictions
  • Bachelor of Science in Nursing required
  • Current BCLS-HCP Certification
  • Demonstrated clinical experience in providing clinical oncology and oncology-related care 
  • Demonstrated clinical experience in medication administration and management 
  • Demonstrated experience in clinical informatics
  • Expertise in cancer survivorship, chronic disease management, and behaviour change strongly preferred
  • Ambulatory experience preferred
  • Virtual care and/or remote monitoring experience an asset 
  • Geriatric care experience and management of co-morbidities an asset
  • Mental health experience an asset
  • Experience in change management and ability to adapt to a constantly changing environment that requires ongoing re-evaluation of priorities
  • Strong working knowledge of adult teaching principles relevant to supporting patients in gaining the knowledge, skills, attitude and self-awareness necessary to effectively manage their chronic condition(s)
  • Strong organizational and time management skills
  • Team player with excellent communication, critical thinking and customer service skills 
  • Supportive of the patient-centered experience, your colleagues and others' cultural and spiritual beliefs
  • An innovative thinker with vision and dynamic problem-solving skills 
  • Excellent verbal and written communication skills
