THE ROLE: Client Care Coordinator

Type: Full-Time, Including Evenings & Weekends, On-Call, On-site

Key Responsibilities:

  • Coordinate and manage client care schedules, ensuring coverage and continuity of care
  • Communicate regularly with clients, families, and staff to assess needs and resolve concerns
  • Maintain accurate client files, documentation, and care records
  • Match caregivers to clients based on skills, compatibility, and availability
  • Support recruitment, onboarding, and orientation of new care staff as needed
  • Monitor client satisfaction and participate in quality assurance initiatives
  • Collaborate with management to ensure compliance with company standards and health regulations
  • Uphold client satisfaction, confidentiality and high quality care

Role Requirements:

  • Minimum 3 years experience as a Client Care Coordinator in a healthcare setting or in an administrative or clinical role
  • Excellent communication, multitasking, and problem-solving skills
  • Strong organizational and computer skills (Microsoft Office, scheduling software, etc.)
  • Compassionate and professional demeanor with a focus on client satisfaction
  • Ability to work independently and as part of a team
  • Knowledge of home health care, safety measures, language and administration policies
  • Proficiency in computers and experience using AlayaCare Software is preferred
  • Strong work ethic, positive attitude, commitment and flexibility

  • Vulnerable Sector Screening Police Check
  • CPR First Aid
  • Updated Immunization Records (TB, Hepatitis, etc.)
  • Proof of COVID-19 Vaccination & most recent COVID-19 test result
