Hospital Discharge Social Worker (Part-Time, Casual) — $60/hour

We are seeking an experienced Hospital Discharge Social Worker to join our team on a part-time basis. The ideal candidate has hospital experience in discharge planning and understands the importance of safe, well-coordinated transitions of care. This role offers flexible hours.

Key Responsibilities:

  • Support patients and families in planning for treatment.
  • Assess patients' needs and connect them with rehabilitation programs.
  • Assist with applications for benefits, and other support services.
  • Communicate effectively with other stakeholders to ensure continuity of care.
  • Maintain accurate documentation in accordance with Q Medical standards.

Qualifications and skills:

  • Master's degree in Social Work (MSW) from an accredited program.
  • Registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Minimum 2 years of experience in hospital discharge planning.
  • Knowledge of Ontario healthcare system and community resources.
  • Strong interpersonal, communication, and advocacy skills.
  • Ability to work independently while collaborating within a multidisciplinary team.
