Founded to provide compassionate, clinical, end-of-life care, The Hospice of Windsor and Essex County has been caring for our community since 1979. Our Hospice believes in equitable access to care for all. We provide specialized wrap-around services to adults, children, and their caregivers, in the setting of their choice – whether it be in our Residences or in their home.
With only partial government funding, it has been through the generous support of our volunteers and our community that we have continued to provide memorable care, meeting the physical, emotional, and spiritual needs of patients and their loved ones.
ROLE OVERVIEW
This role will appeal to a caring and compassionate individual who values hospice palliative care and who is eager to support the organization and join an energetic team. The Personal Support Worker (PSW) is dedicated to the vision, mission, and values of Hospice, and puts their best efforts forward to promote the interests of the organization as well as contribute to a positive work culture.
The PSW of the Hospice Residences (Windsor and Erie Shores) will support and work towards achieving the strategic directions set out by the Board of Directors of the Hospice of Windsor and Essex County. Hospice's strategic directions, mission, vision and values will be the basis of annual employee performance evaluation.
This role will function as a front-line PSW, working alongside registered nursing staff, supporting the Hospice Residence Manager in fulfilling the needs of the residence.
PRIMARY DUTIES AND RESPONSIBILITIES
- When a patient is no longer able to contribute to their care, continue to provide care in a way that maintains the patient's dignity, well-being and self-image.
- Work alongside registered staff including Registered Nurses (RN) and Registered Practical Nurses (RPN) to provide specialized care as specified in the care plan, according to protocols, within scope of practice and in accordance with Hospice's policies and procedures.
- Communicate and collaborate effectively as a member of the Hospice Residence multi-disciplinary team which includes but is not limited to RPNs, RNs, physicians, Social Work, Spiritual Care, leadership and volunteers.
- Provide support for Volunteer services as required, including taking orders and delivering meals.
- Document care provided in patients' electronic medical record.
- Maximize patients' participation in their own care, and enable choice to the fullest extent possible. When a person refuses assistance, pursue a balance between respect for the patient's right to choose, and the need to provide a minimum standard of care.
- Continuously monitor patients' daily physical, emotional and psychological functioning and adapt approaches within the PSW scope.
- Demonstrate knowledge of the stages of dying.
- Recognize and report changes in patients' level of pain and distress, signs of active dying; communicate changes in patient needs and condition to registered staff.
- Provide post-mortem care at the time of death, in accordance with patients' wishes.
- Support the Hospice Residence Manager with daily management of the residence including administrative tasks as required – e.g. stocking supplies, setting up patient rooms.
- Carbolize rooms to prepare for patient admissions.
- Communicate and implement Hospice's support services as appropriate for patients and families.
- Demonstrate openness and sensitivity to social, spiritual and cultural values and practices that may influence individuals and family preference and ensure beliefs are acknowledged are accommodated.
- Work in collaboration with the Coordinator of Volunteers to ensure adequate volunteer services within the residence.
- Accurately assess and manage individuals' and families' loss, grief and bereavement needs, and refer to members of the interdisciplinary team as needed.
- Understand how a Substitute Decision Maker (SDM) is determined (based on the hierarchy within the HCCA) and the role the SDM plays in health care decision making if a person is not capable.
- Welcome and orientate new patients and families into the Hospice Residence, in order to achieve a fulsome understanding of the environment and support system available within the home.
- Provide orientation and mentorship to any new staff and students within the organization to ensure they are well orientated to their roles within the organization.
- Complete thorough patient report/communication huddles at the prescribed times during the shift.
- Maintain a safe environment for residents, families/visitors, staff and volunteers by completing ongoing surveying of the environment and actively addressing issues as they are identified and/or reported.
- Communicate, implement and ensure that Hospice's responsive and holistic culture is maintained and evident within the residence.

REQUIRED QUALIFICATIONS AND EXPERIENCE
- TECHNICAL REQUIREMENTS
- Legally entitled to work in the province of Ontario
- Evidence of a Vulnerable Police Clearance completed within the past year
- CPR Certification and annual maintenance
- Ability to speak fluently in the French language is an asset
- Experience in the use of a computer, including programs such as Cerner, Microsoft Word, Excel, and Outlook as a minimum
- Excellent oral and written communications skills
- Personal phone is required for scheduling, payroll and MFA application access
- EXPERIENCE
- Minimum 1-2 years of PSW experience in a comparable setting (e.g. long-term care, complex care)
- Experience in oncology and/or palliative care is preferred
- EDUCATION
- Completion of a PSW certificate from an accredited college.
- Completion of the "Fundamentals of Palliative Care", OR completion of Hospice's "Direct Service Volunteer Training" course with a plan to complete the "Fundamentals of Palliative Care" within one year following the start date of employment
- Completed or are willing to take "Learning Essential Approaches to Palliative and End of Life Care" (LEAP) course preferably within one year of employment.
