- Supporting the design, implementation and dissemination of strategic initiatives and program services, in alignment with evidence-informed approaches.
- Assisting in quality improvement efforts to improve current and emerging client care and service delivery.
The Lead will support the design and implementation of strategic initiatives, primarily focused on advancing the services and support for the Loop Network and community partners. Key responsibilities include:
- Building relationships with community members by fostering a welcoming, supportive online environment for individuals affected by lung disease, their families and caregivers.
- Facilitating virtual support groups, discussion forums, and peer-to-peer engagement, encouraging active participation.
- Cultivating relationships with community organizations and individuals with the objective of building mutually beneficial partnerships.
- Coordinating activities which include set-up, administration, tracking and monitoring of participant registration and attendance, program schedules, and evaluation instruments.
- Representing the department at meetings and community events.
- Assisting individuals affected by lung disease to inform the development and improvement of community-focused initiatives.
- Collaborating with the LHF team to devise evidence-informed approaches to design programs/ services and to collect data for ongoing program services and to support organizational initiatives, e.g., online surveys, interviews, group meetings.
- Collaborating with cross functional teams to manage social media and website updates to promote and disseminate resources to the larger community.
- Supporting the evaluation process of programs and services.
Key expectations include:
- Applying a health equity lens to project development and analysis
- Supporting the creation of inclusive materials and programs
- Raising equity considerations in program planning and evaluation
- Fostering inclusive collaboration within cross-functional teams
- Support and contribute to additional programs or initiatives based on current and emerging priorities and needs;
- Help grow and maintain LHF's network of community partners and people with lived experiences (e.g., living with lung conditions, nicotine experience, caregivers)
- Support cross-functional duties aligned with organizational goals
- Assist in external communications and advocacy efforts, including media engagement in collaboration with the Marketing and Communications team
- University Degree or graduate degree in public health, health sciences, life sciences, or a related field; graduate-level training is considered an asset.
- Experience working in respiratory health, primary care, or health systems considered a strong asset.
- Minimum three (3) years of experience in:
- Supporting people with lived experience (i.e. patients, caregivers, public)
- Health promotion, program implementation, chronic disease management
- Stakeholder engagement and co-design in programs and services
- Strong understanding of evidence-based practice, quality standards, and patient-centered care.
- Demonstrated experience with project coordination, community development, outreach and partner relations.
- Fluency in French is considered an asset.
