About the job

LTCH Homes Assessment & Triage Officers are the first point of contact, promoting the health, safety, care, and rights of Long-Term Care Home residents. Every day will be a little different, but you can expect it to include activities like:

  • gather, document, assess and triage information (concerns, complaints, and critical incidents) received regarding Long-Term Care Homes in Ontario
  • establish the severity of the information received and outline the associated risks to residents
  • create the inspection program's response and action
  • support and conduct inquiries
  • prepare correspondence, briefing notes and other materials
What you bring to the team
Mandatory requirements
  • Current registration as a Registered Practical Nurse (at minimum) with the College of Nurses of Ontario
  • Willingness to travel small distances on a periodic basis for training and education
Communication, Listening and Interviewing Skills:

You have the ability to:

  • deal with contentious issues, sensitive personal health information and complaints from upset or angry individuals
  • effectively communicate with the residents, complainants, stakeholders, the public and LTC Home staff
  • communicate with persons about complex health care issues
  • develop correspondence and briefing notes utilizing your written communication and computer skills
Clinical Nursing and Assessment Skills:

You are able to:

  • assess information gained from discussions and interviews regarding the care and services provided to residents
  • determine if there is a true risk of harm to residents
  • discuss medically complex situations and current clinical best practices with other health professionals
  • determine what legislative requirements may be in non-compliance
Knowledge of Health Care Operations:

You have demonstrated knowledge of:

  • long-term care homes and health care system operations
  • current health care practices (e.g. medication administration, skin and wound care, pain management, responsive behaviours, etc.)
  • relevant acts and regulations governing Long-Term Care homes to interpret, apply and ensure compliance (e.g. Fixing Long-Term Care Act, 2021, the Provincial Offences Act, etc)
Problem-Solving and Conflict Resolution Skills:

You have the ability to:

  • appropriately respond to complaints about resident care and services
  • prepare ministerial responses to resident care issues
  • diffuse and resolve contentious and highly sensitive issues
  • work independently and make logical decisions in an emotional, high stress environment
  • work collaboratively within a provincial team
