Purpose: In collaboration with the Director of Pharmacy, the incumbent will support the development and implementation of quality and patient safety programs, diversion surveillance program, medication reconciliation services, and related policies and practices within the Pharmacy department. The incumbent will seek solutions to improve medication system performance and align Pharmacy services with SPARQ Strategic Plan.   Duties:
  • Collaboration with Pharmacy Leadership to perform administrative duties related to pharmacy quality, implementation or training, and audit frameworks. (e.g. developing standard operating procedures to improve medication system performance and dissemination of best practices, performs audits and provides feedback to ensure data integrity and quality standards are met, participation in regulatory visits and accreditation processes, providing expertise and support to related committees and working groups)
  • Support Medication Safety Working Group
  • Facilitate a controlled substance surveillance program including conducting audits, narcotic tracers, coordinating working group meetings. 
  • Identify process enhancements that improve accuracy and efficiency of pharmacy medication reconciliation services to ensure accuracy and compliance with quality standards. 
  • Coordinating changes in medication reconciliation process with the implementation of EPIC. 
  • Compare and analyze statistics related to medication reconciliation team performance, identifying trends and areas for improvement 
  • Ensuring Ontario College of Pharmacists and National Association of Pharmacy Regulatory Authorities' standards related to the practice of compounding is upheld.  
  • Responsible for training, standards of practice/procedure development, and audits related to compounding 
  • Coordinating special projects assigned by the Director of Pharmacy and contributing support to achieve project objectives 
  • Work in a manner that follows staff and patient/client safety practices, policies and procedures of The Royal.

  • Strong knowledge of practice standards applicably to hospital pharmacy practice, including Accreditation Canada, Ontario College of Pharmacists, and National Association of Pharmacy Regulatory Authorities 
Qualifications:
  • Graduate of an approved Pharmacy Technician course from a recognized community college. 
  • Registration and in good standing with the Ontario College of Pharmacists.
  • Registered Pharmacy Technician (RPhT) 
  • Pharmacy technician in a hospital setting 
  • English level A- is mandatory in oral expression, oral comprehension, reading and writing.  Bilingual (French/English) is considered an asset.
