We are searching for a Physiotherapist to join Shannex Transitional Health Services at West Bedford Transitional Health team based in West Bedford, Nova Scotia
Through a new, innovative partnership with Nova Scotia Health (NSH), Shannex proudly operates the province's first transitional health facility in West Bedford. The facility provides specialized care for NSH patients in the central zone who no longer require hospital care but need additional support and services before safely returning home or to their community.
Reporting to the Allied Health Manager, the Physiptherapist is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decision-making, and mutual support.

Part of the allied health team, working in collaboration with Physiotherapists, Occupational Therapists, Recreation Therapists, Recreation Programmers, Social Workers, Dietitians, RNs, LPNs, care team, and Nurse Practitioners, to support patients' functional goals for discharge.Responsible for prioritizing and managing own caseload; participates in the patient's overall plan of care, providing consultation, assessment, intervention, evaluation, and follow-up while working within the site setting, the patient's home and/or their community.Assesses, diagnoses, and treats patients who present with actual or potential impairment, pain, functional limitations or disabilities. Physiotherapy assessment involves obtaining a detailed medical history, performing specific and standardized tests and evaluation procedures to determine a physical diagnosis.Promotes active participation of the patient, family, or caregiver in establishing physiotherapy goals for discharge, which are based on assessment findings, prognosis, and evidence-based practice. Selected interventions are implemented to achieve and maintain optimal health, fitness, and function; encourage self-management of identified impairments and disabilities and relieve or manage painWorks in collaboration with the patient, caregivers, family, and interdisciplinary healthcare team to facilitate, restore and/or maintain function to support discharge back to the community. Provides patient education to support discharge plan and wellnessProvides supervision and assigns tasks to OTA/PTAsParticipates in continuing education to maintain credentials and increase expertise and knowledge.Ensure new team members are orientated and trained in all departmental standards.Documents in patient health care record in accordance with established standards.Communicates appropriately and effectively, through verbal, nonverbal, written, and electronic means, with the patients, Substitute Decision Makers, OT, PT, other members of interdisciplinary team.Additional duties as requireAbout You
A master's degree in Physiotherapy Current registration with Nova Scotia Regulator of Physiotherapists; Minimum of two years clinical experience; previous experience in acute care, rehabilitation and discharge planning preferredKnowledge of the principles, methods, techniques, materials, equipment, and procedures used in physiotherapy and their application in specific cases. Knowledge of skeletal, muscular and neuro anatomy and physiology as it applies to physiotherapy.Knowledge of the potential hazards involved in physiotherapy treatments and precautionary measures to be taken. Ability to plan and organize development of restorative treatment goals and objectivesAbility to accurately observe and assess progress of patients being treatedAbility to instruct staff in the proper application of routine physiotherapy restorative action plans.A passion for the healthcare sector, home-first and/or ensuring patients reach functional outcome goals and maximize quality of lifeAbout Us
