We are seeking a dedicated Specialized Service Coordinator to join our team. In this role, you will be responsible for overseeing the delivery of specialized services to individuals, ensuring their needs are met efficiently and effectively.

KEY RESPONSIBILITIES AND DUTIES:
The Individualized Supports Coordinator will report directly to the Director of Community Services and will collaborate closely with third party clinical teams and community partners to ensure comprehensive and effective support strategies:

- Work with Director of Community Services, managers and staff teams to build a program that will result in a person-centred comprehensive quality of life (QOL) support programs that encompasses clinical, bio-psycho social framework, applied behavioural analysis and choice/inclusivity including reporting and metrics to ensure success.
- Invest additional time to develop a deep understanding of individuals supported by the agency, trends, resources and treatment options particularly those with behavioral and mental health needs, through meaningful interactions and engagement.
- Offer expert consultation, coaching, and guidance to program managers and staff teams on effective approaches for supporting individuals with diverse needs, leveraging specialized knowledge and experience to enhance service delivery and outcomes.
- Participating as a member of a multi-disciplinary team; acting as a liaison internally and externally in order to promote a greater understanding of the needs of individuals with developmental disabilities and to expedite service; cultivating a progressive philosophy with respect to disability-related issues within the agency and Community.

Qualifications:

- Demonstrated belief in CLGS’s Mission, Vision, and Values.
- Thorough working knowledge of the Strategic Plan.
- Excellent communication skills, both verbal and written.
- Bachelor’s degree in psychology, Socia Work or another human service-related field
- Minimum of three years working with individuals with developmental disabilities
- Ability to work in a dynamic environment requiring exceptional organizational ability, sound judgment, flexibility, patience, and independent decision-making.
- Bachelor’s degree in psychology or equivalent human services field.
- Minimum 3-5 years of experience working with individuals with developmental disabilities.
- Minimum 3-5 years of experience in the development and implementation of Behavior Support Plans.
- Proficiency in French is an asset.
