Recovery Advocate

Reporting to the Health and Wellness Manager, the Community Based Advocate is a member of a multidisciplinary team on a recovery transitional housing program. The program is based on a sober recovery model, with intakes taking place on a rolling basis. The program has transitional housing supports, which will focus on building recovery capital in multiple areas such as skills for independent living, financial literacy, employment, and connection to long-term/post program community-based supports such as mental and physical health, ongoing recovery programs, social and recreation programs, cultural supports and others. The Community Based Advocate supports discharge planning and provide follow-up supports.

**MAJOR DUTIES AND RESPONSIBILITIES**:
**Major Duties**
- Process referral received from Coordinated Access and Assessment (CAA) to facilitate appropriate move-ins to the program, up to and including liaison with other agencies and treatment centers
- Provide updates to CAA on the status of referrals
- Educate and empower clients in the recovery program to understand and achieve their rights and responsibilities, as participating members of society
- Liaise with internal and external resources that will help clients increase their assets. This includes maintaining existing partnerships as well as developing new partnerships
- Provide case management for up to 10 clients, including tracking progress on a quarterly basis, facilitating access to internal and external referrals, identifying needs, and encouraging steps towards housing sustainability
- Support facilitation of programming as necessary
- Provide crisis intervention, as necessary, or connect to other crisis supports
- Provide input into program evaluation upon request
- Educate and empower clients to understand and achieve their rights and responsibilities, as participating members of society
- Other duties as assigned.
**Administration**
- Build a comprehensive library and knowledge base on community resources
- Share and disseminate community resource information
- Track assigned clients’ progress with external resources
- Document client interactions, client referrals and client progress on HMIS
- Track statistics and prepare reports, as required
- Attend meetings and training, as assigned
- Serve on internal and external committees, as requested
- Provide input into the development of policies and procedures for designated programs
- Keep clients’ records and information confidential, updated and secure
**MINIMUM QUALIFICATIONS AND SKILLS**
- BSW or Bachelor in Psychology or equivalent
- Three to five years of related working experience
- Competent in use of MS Office
- Excellent interpersonal skills
- Excellent administrative, time management and organizational skills
- Self-motivated and ability to work independently
**WORKING CONDITIONS**
- This position is primarily an office position.
- Time more than regular working hours may be required on occasion.
- Local travel to various Mustard Seed sites and external agencies will be required
- Employees are to be aware that many of our clients may experience difficulties or impairment at times, which could result in them uttering threats, discriminatory statements, and other insults at your person. This may in a raised voice, or in seeing unpredictable behaviour up to and including the possibility of them becoming aggressive.
- Training on how to respond is provided, but this is to be considered typical working conditions. Many of our clients are unaware of the impact of these comments.
