**Job Summary**:
The Right at Home Canada Care Planner is the key liason between the community and our Home Care Services. Our Care Planners function as the “Care Concierge” that match the family’s needs with a customized Care Plan and caregiving team.

The Care Planner is a crucial member of the Care Team. They are primarily accountable for all aspects of care delivery - handling care inquiries, service assessments, care planning, care team formation
- all of which impact the overall client / family experience.

This is a hybrid position working predominantly remotely, but there is also an in person requirement (training, client consults, team meetings, PPE days, community events, etc).

**Qualifications**:

- A minimum of 2 -3 years Care Planning experience in the healthcare field
- Nursing and or a Clinical Healthcare background preferred
- Familiarity with medical terminology
- Experience with InflowCare Inquiry software and AlayaCare Home Health Care software preferred
- Ability to multitask while working in a fast-paced, changing environment
- Ability to organize and prioritize work effectively
- Ability to work both independently and as part of a team
- Ability to express ideas clearly in both written and oral communications
- Excellent interpersonal skills
- Proficient with Microsoft Office & computer skills

**Responsibilities**:

- Responds to inbound care inquiries and conducts client assessments (on the phone or in person)
- Acts as a key resource to client and client's families
- Develops customized client Care Plans and modifies as client needs change
- Communicate the Care Plan to the Care Team
- Oversees and supervises the care delivery
- Liaises with Care Coordinator/Scheduler to match caregiver availability to client needs
- Recruits caregivers (PSWs, RNs, RPNs, Companions)
- Networks across key stakeholders to maintain and develop professional and social contacts (hospitals, discharge nurses, retirement homes, community/national associations - Alzheimer’s Assoc, etc)
- Attend marketing meetings, presentations, and targeted community awareness events as appropriate
- Conduct and facilitate community presentations
- Participate in relevant community events (fundraisers, charity walks, bbqs, etc.)
