Family Resource Network Navigator

YMCA of Northern Alberta is looking for a Family Resource Navigator to provide direct service delivery to eligible percipients and is responsible for the effective planning, implementation and evaluation of program services, they will be working with diverse populations who face unique challenges. The Family Resource Navigator will provide services through in-reach and outreach approaches to best meet the needs of families and to reduce the barriers of support.

**Position Responsibilities**
- Plan, implement, evaluate services/workshops.
- Triage families, follow case management practices and assist with transfers of participants to internal and external programs.
- Collect necessary information to confirm eligibility and complete participant intake and/or assessment to inform ongoing participant supports.
- Support participants.
- Engage with community agencies, and other organizations to maximize services and resources to participants.
- Maintain knowledge of relevant services and community resources related to the Social Determinants of Health.
- Complete administrative tasks such as inputting data and maintain participant files accurately and in a timely manner.
- Collect data, including outcomes to inform and contribute to the development of funder and YMCA reports
- Plan and work within program budgets.
**What You’ll Need for This Job**
- University Degree in Child and Youth Care Counselling, Human Services field or equivalent.
- 2 years’ experience working directly with youth and/or young adults with multiple risk factors
- Experience working with diverse populations
- Valid driver’s license and vehicle.
- Non-Violent Crisis Intervention and Suicide Intervention Certification are preferred
- Standard First Aid & CPR-C
- Able to obtain a Police Information Check and Vulnerable sector and Child Intervention/Enhanced Security check
