Housing Intake Worker

**Hours**: Monday-Friday 40 hours/week | Fulltime Permanent
**Location**: 316 Spruce Street South, Timmins ON
**Position Summary**:
Under the general direction of the Senior Management Team, the Data Management Assistant is responsible for operating the day-to-day intake and screening of all clients accessing Living Space and will ensure intakes are completed and distributed to the appropriate program for action. The Data Management Assistant will also be responsible for supporting the intake process for the Cochrane District System of Care’s Homeless By-Name List (BNL) referrals.
The Data Management Assistant will conduct intakes (in-person, video, or phone), risk and vulnerability assessments for all new and existing clients of Living Space and the Cochrane District System of Care. The individual will be responsible for preparing and maintaining all appropriate administrative records.
**Requirements**:
- Paid or unpaid experience working with vulnerable individuals.
- Post-secondary degree or diploma in Systems Development, Community Development, or related field. A combination of relevant education and work experience will be considered an asset.
- 1-3 years experience in a relevant field.
- The ability to engage and work with a broad range of clients with diverse and complex issues.
- Demonstrated critical thinking skills including strong clinical judgement and conflict resolution skills.
- Knowledge and understanding of the complex issues related to addictions, mental illness, poverty, and homeless, as well as the impact of social/cultural/economic factors on the determinants of health.
- Experience with electronic reporting and case notes is considered an asset.
- In-depth knowledge of social service, housing, and support services in Timmins and the Cochrane District required.
- A satisfactory Vulnerable Sector Criminal Records Check is required.
- Living Space has a Vaccination Policy that requires employees to take part in COVID-19 vaccination programs recommended by Public Health and provide an up-to-date TB screening record.

- Demonstrated knowledge of issues related to poverty, homelessness, substance use, and mental illness.
- Demonstrated ability to work independently in community settings with mínimal supervision.
- Demonstrated ability to develop a wide range of high-quality written materials. This includes but not limited to case notes, shift logs, incident reports, legal documents, and electronic documentation.
- Proven effective time management skills.
- Demonstrated strong sense of initiative, personal accountability, and dependability.
- Familiarity with community partners and the programs they offer.
**Specific Duties**:
- Conduct initial intake assessment using established criteria, risk assessments, and safety planning.
- Directly support the maintenance of the Cochrane District System of Care’s By-Name List (BNL).
- Follow up with high-risk clients who have registered for services and are awaiting connection to services.
- Provide phone coverage of the intake lines during business hours.
- Update agency database of client enrollment information, including demographic data.
- Keep careful and accurate records and maintain client case files of all relevant information.
- Support the provision of appropriate data and data reports to support monthly reporting for intake, housing, and shelter use reporting.
- Support the Community Housing Worker with administrative tasks and other duties as required.
- Complete follow up home visits to clients in the community. Check-ins will vary case by case.
- Ensure proper documentation to monitor client progress. Include any recommendations for supports that will benefit the client.
- Communicate with the Community Housing Worker on recommendations based off home visits.
- Work closely with community partners to assist in the delivery of their programs i.e., remind clients of partner appointments, help manage calendars for client.
