**Summary**

The Support Worker role will support individuals with addiction or concurrent disorders within a hospital setting by promoting understanding, acceptance, and advocacy. The hospital teams, consisting of an addictions medicine physician, a social worker, and an Umbrella Peer Support Worker, offers consultations, assessments, and treatments. The Umbrella outreach worker collaborates with the social worker and physician to assist with discharge planning, hands-on patient support, and connecting patients to community services

**Responsibilities**
- Provide one-on-one support to clients facing substance use and mental health challenges.
- Welcome and engage with clients who appear on the AMCS or ER/PES list.
- Assist clients in developing personalized recovery plans and goals.
- Assists with referrals to treatment centres and community resources as needed.
- Maintain accurate case management records and documentation.
- Collaborate with other professionals on the AMCS or ER/PES team to ensure comprehensive care for clients.
- Advocates for patients and acts as a liaison between clients and hospital staff.
- Offer community, clinical, and non-clinical options for clients and their care providers/families.
- Maintain and update the handover list on the shared drive with client progress notes
- Collaborate on patient planning with AMCS or ER/PES Social Worker, Doctor, and unit Social Workers.
- Monitor client progress and adjust support strategies accordingly.

**Required Knowledge, Skills, Abilities, Other characteristics (KSAO)**:

- Strong preference will be given to applicants with lived experience in substance use disorders and recover
- Strong interpersonal skills, with the ability to connect with a variety of personalities
- Knowledge of harm reduction principles and practices
- Excellent time management skills to prioritize tasks effectively.
- Prior training and experience in trauma-informed care considered an asset
- Prior experience working in the field of addictions is preferred.

**Job Types**: Part-time, Casual

Pay: From $26.50 per hour
