Do you have experience in a Community-based setting and have developed the skills, expertise and knowledge to successfully interact, engage and provide care for clients with moderate to severe Alzheimer Disease and/or related dementia?

Providence Healthcare offers a unique, community-based, Adult Day Program, 24 hours a day, 7 days a week. Our home-like and social program operates 365 days a year, including statutory holidays and offers flexible hours, caregiver relief and meaningful activity for individuals with a moderate to severe dementia.

As a member of the Adult Day Program interdisciplinary team, the Clinical Resource Coordinator is responsible for providing direct service-delivery for our clients who attend the Adult Day Program, through service coordination, personal care and recreational programming, in accordance with Providence Healthcare’s Mission and Values.

**REQUIRED SKILLS**:

- Minimum 1 years’ experience working with individuals with Alzheimer’s Disease or related dementias
- A thorough understanding and working knowledge of Alzheimer disease and related dementias
- Trained in dementiaAbility©, Positive Approaches© to Care and/or U-FIRST/PIECES© preferred
- Demonstrated ability to manage high volume and varied work assignments with accuracy, flexibility and in a timely fashion
- Ability to work independently under pressure and establish priorities under time constraints
- An understanding of recreation, activation methodologies, principles and practice
- Previous experience with planning, implementing and evaluating therapeutic programs and activities
- Skilled in being able to provide personal grooming, bathing, incontinence care, toileting and nutritional assistance for clients with an irreversible dementia
- Self-motivated with demonstrated ability to function as a part of a team and/or independently, creative, and exhibits a good sense of humour
- The ability to work independently, be creative, and flexible
- Strong sense of caring and compassion
- Experience in a community-based setting preferred
- Strong organizational skills, problem-solving skills and critical thinking skills
- Excellent oral, written and interpersonal and communication skills
- Knowledge of client data base software systems such as HiNext TREAT preferred
- Knowledge of the LHIN Home and Community Care processes
- Strong organizational, interpersonal, team collaboration and communication skills
- Knowledge and sensitivity to work in a culturally diverse environment
- Very good past performance
- Very good attendance record
