**What You’ll Do**:

- Conduct resident pre-admission, admission, and ongoing assessments to identify goals, strategies, and expected outcomes.
- Participate in interdisciplinary team conferences.
- Serve as a liaison with families, providing guidance and support.
- Offer counselling or group sessions for residents and families (e.g., orientation, adjustment programs, crisis support, bereavement counselling).
- Act as a resource for staff seeking guidance, referrals, or support.
- Plan and provide in-service programs for staff, residents, and families.
- Maintain accurate records, reports, and statistics in accordance with policies, procedures, and legislation.
- Participate in committees related to resident care and facility operations.
- Foster open communication with residents, families, and team members to ensure resident needs are met.
- Liaise and consult with government officials and professionals regarding resident care.

**What We’re Looking For**:

- Social Services Worker (SSW) Diploma from an accredited college; bachelor’s in social work (BSW) is an asset.
- Strong knowledge of community resources, aging, gerontology, and dementia care.
- Previous experience in long-term care preferred.
- Familiarity with legislation, counselling techniques, and group sessions.
- Knowledge of occupational health and safety practices.
- Strong English reading, writing, and communication skills.
