As a **Psychosocial Spiritual Bereavement Clinician**, you will be concerned with the psychological, emotional and spiritual well-being of the persons who are dying, their partners and their families. There is also a need for support with the necessary practical aspects of end-of-life decision making. The Psychosocial, Spiritual, Bereavement Clinician can provide a wide range of interventions including individual, couple and family counseling, advocacy, education, access to community supports, and spiritual companioning. The Palliative Care Team works within a shared care model with family physicians, community nursing agencies, hospital and hospice staff and community volunteers, providing that shared expertise and resources make for a more effective and efficient workforce.

Core Duties - Responsibilities:
- Receive referrals from either the front line primary care team providing palliative care, or from colleagues on the consult team
- Provide education as it relates to the grieving process and bereavement support; explain the process of normal grief and the multidimensional aspects and domains of grief (emotional, physical, cognitive, behavioural, social and spiritual)
- Identify the need for further in-depth counseling and arrange the appropriate referrals
- Promotes patient safety in alignment with the Vision, Mission, Values and Strategic Directions of the HCCSS
- Identifies work stressors on the teams, as well as those stressors present on cases or in the work environment in order to collaborate with the consult team on the development of
- Provides effective documentation and ongoing statistics
- Participates with community colleges and universities in the development of hospice palliative care inter-professional educational programming
- Participates at rounds, conferences and other educational forums as appropriate and as required
Qualifications:
- Minimum Master’s Degree in one of the following disciplines: Social Work, Educational or Counseling Psychology, Nursing, Divinity and Spirituality, Theology or other academic preparation as assessed by hiring team
- Current registration with applicable college
- Training in counseling through CAPE or equivalent counseling and supervision program
Skills & Abilities:
- Evidence of continuing professional development
- Proven leadership abilities with effective skills in verbal and written communication, problem solving and team building
Other:
- Must have valid driver’s licence and ability to drive and use own car
- Other duties as assigned
- Location: Position will be based out of the Hamilton Branch; however, time will be split between palliative clients in the Hamilton and Haldimand Norfolk areas. Time between branches is self-scheduled according to client need
