We are currently looking to hire an:
**Individual Support Worker - ABI Outreach Team**
**1 Permanent Full Time (1.0 FTE - 35 hrs/wk)**
**About the Position**
We require one full-time experienced Individual Support Worker to work as team member of the ABI Outreach Team. The ABI Outreach Team is an interdisciplinary team of professionals that supports people with an Acquired Brain Injury, as well as their families, in their homes and other community settings as needed. The team is composed of professionals who work cohesively as one team to support the complex healthcare needs of this client population, their families and caregivers.
**Reporting To**:Manager, Community Health and Support Services
**Key Areas of Responsibilities**
- To provide training, coaching & support to help clients learn skills that will increase their independence in the community
- Provide recovery-oriented community-based support to individuals living with acquired brain injuries
- All activities are performed to encourage client independence and promote client hope, self
- determination and decision-making
- Assist with the completion of individual client Support/Goal Plans: o Provides input into assigned clients' Support/Goal Plans; o Shares responsibility with the Team for implementing supports that assist clients in pursuing/achieving their goals; o Assumes a primary role in teaching clients to achieve their care goals
**Documentation and Administrative tasks**
- Properly document all client interactions and meet the both organizations documentation requirements
- Participate in regular team meetings and program activities, as required
**Other Responsibilities**
- Works cooperatively, respectfully and constructively with clients, families, team members, other service providers, volunteers, management, and external stakeholders
- Ensures confidentiality of client, employee, and program information is maintained at all times
- Undertakes other related and/or delegated duties as may be assigned/required
- Attend all training related to this position and actively participates in these sessions to the satisfaction of the instructors
**Educational and/or Professional Qualifications**
- Valid Crisis Prevention (CPI), First Aid/CPR, and ASIST certificates preferred
**Experience**
- Relevant experience in supporting clients with special needs, physical and/or cognitive disabilities, mental health and or addiction issues
**Judgment**
- Ability to identify client risk factors and coordinate appropriate follow-up
- Good judgment
- Respectful and aware of client-worker boundaries
- Awareness of safety issues with clients
**Significant Working Conditions**
- Conduct visits in the clients’ homes, hostels, hospitals and other community settings working in sometimes difficult conditions, including hoarding and pest infestations
- Work cohesively and take direction from staff members from both Cota and SCHC in accordance with the shared vision and protocols for the program.
- Regular travel throughout Scarborough for client visits and other meetings at various sites.
- Provide proof of a valid Ontario Driver’s license and evidence of a clean driver abstract
- Access a vehicle for daily use
- Be available to work during the day time hours; however, evening and weekend shifts may be available or required on occasion.

**Remuneration**: $19.35 - $20.85
**Pay Band: J**
