Community Social Worker

(Full-Time)

At Community Care Durham, coordinated networks of volunteers and inter-professional staff, working out of integrated community hubs and other locations, strive to enrich the well-being and quality of life for people who choose to live at home. As the largest community support services agency in Durham Region, CCD collaborates with its partners and the broader community to address community issues and support local citizens to achieve their goals to live independently.

Reporting to the Vice President of Client Services, the Community Social Worker will provide social worker services across the Durham Region supporting clients of the Adult Day Program (ADP), Supportive Housing (SH), Assisted Living Services (ALS), and other Community Support Services (CSS). The Community Social Worker provides essential social work services to clients in various CCD programs, addressing both their social and emotional needs. In this collaborative role, the Community Social Worker will work closely with CCD departments, community organizations, and external agencies to support clients, and provide them with resources to live fulfilling lives. The Community Social Worker’s efforts are vital in promoting the well-being of our clients and enhancing their quality of life.

Responsibilities include but are not limited to:

- Conducts thorough client assessments, identifies needs, and provides tailored support to individuals across multiple CCD programs;
- Participates in the personal development of clients to improve their quality of life;
- Provides short-term counselling services to CCD clients, helping them navigate personal challenges and develop coping strategies to improve mental and emotional well-being;
- Develops realistic, client-centered goals and care plans;
- Ensure ongoing support and monitor client progress, adjusting care plans as needed to meet evolving needs;
- Ensures the safety and well-being of oneself, clients, and colleagues through professional practice and adherence to safety protocols;
- Advocates for clients’ needs, and connects them with appropriate resources and services that align with their unique circumstances;
- Maintains an awareness of community resources, recommends strategies to bridge identified gaps and facilitates connections between clients and community resources;
- Provides education to clients and caregivers, offers guidance and links them to community agencies and support services when appropriate;
- Provides services that respect and reflect the cultural values and needs of diverse populations within the community;
- Conduct in-home visits and follow ups as needed;
- Completes accurate and up-to-date documentation regarding client assessments, care plans, and service delivery to support the continuity of care, and compliance with organizational standards.

**Qualifications required**:

- Master of Social Work (MSW) or equivalent Master's degree in a related field is required
- Registration with the Ontario College of Social Workers and Social Service Workers in good standing is required
- Three (3) years of experience relative to role is an asset
- Demonstrates knowledge of mental health, and relevant policies and regulations, such as the Mental Health Act
- Knowledge of the legislation, regulations, and policies applicable to the practice
- Experience working in a community setting is preferred
- Proficiency with Microsoft (Word, Outlook, etc.,)
- Is able to manage time and workload effectively
- Has excellent interpersonal skills and ability to build professional relationships with coworkers and clients
- Satisfactory Police Vulnerable Sector Check
- Sensitivity to adults with needs related to aging, physical and/or mental health
- Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

**Compensation**:Starting at$85,000 annually; 3 weeks’ vacation per year of work (pro-rated to hours worked); 9 paid (including stat) holidays, 3 paid float days; health, insurance and pension benefits when eligible.
