**Job Overview**
As a Community Health Worker, you will play a vital role in bridging the gap between healthcare providers and the community. The Better at Home Community Worker provides **non medical** supports to community seniors to assist them to age in place.

**Functions and Responsibilities**
- Performs housekeeping duties such as sweeping & mopping floors, vacuuming, dusting, wash dishes & laundry.
- Shops for groceries and helps with the Meals on Wheels delivery program on occasion.
- Observes clients and their environments & reports unsafe conditions and behavioural, physical and/or cognitive changes to supervisor.
- Demonstrates methods & provides basic information to clients in relation to housekeeping, meal planning and preparation and grocery shopping, in accordance with pre-established care plans.
- Accompanies clients on outings such as appointments, shopping and leisure activities.
- Completes and maintains related records and documentation such as communications books and progress reports.
- Completes a monthly mileage sheet for reimbursement.
- Performs other related duties as assigned.

**Qualifications**
- Grade 12
- Class 5 BC Driver’s License
- Recent, related experience of three months
- Or an equivalent combination of education, training, and experience

Or other qualifications determined to be reasonable and relevant to the level of work
- Must have a cell phone

- Physical ability to carry out the duties of the position.
- Ability to work independently and in cooperation with others.
- Ability to operate related equipment, Ability to communicate effectively, both verbally and in writing
