**Currently we are hiring a Full-Time Lead Engagement Facilitator**

**We are looking for a** Lead Engagement Facilitator which plays a pivotal role in building strong relationships within their designated team. They cultivate a positive work environment and promote opportunities that inspire self-motivation among team members. Acting as Lead Engagement Facilitator, you will uphold the organization’s core values, beliefs, and cultural principles while ensuring that daily operations adhere to established policies and procedures. Your responsibilities include leading, guiding, and supervising teams to successfully implement and oversee personalized services and plans for the individuals they support.

**Primary Responsibilities**:
Ø Ability to be focused on and skilled at engaging participants through meaningful interactions

Ø Implement meaningful, creative and thoughtful programming within the Day Program as well as when out in the community.

Ø Foster relationships built on trust

Ø Learn and utilize communication methods preferred by participants

Ø Comfortable using a portable manual and ceiling Hoyer lift

Ø Assist PSW with lifts, transfers, personal care, feeding (when required)

Ø Ability to work with and support individuals with various abilities, interests, and needs

Ø Communicate with manager any changes in participant condition, incidents, difficult events etc.

Ø Connect/disconnect G tube feeding bags

Ø Complete suctioning when required

Ø Dependable with a strong work ethic

Ø Complete documentation as required

Ø Disinfect, clean and tidy program at end/beginning of day

Ø Follow and adhere to the program Policies and Procedures and Practices

Ø Attend and contribute to regular team meetings as required

Ø Ability to be focused on and skilled at engaging participants through meaningfulinteractions

**Duties are as follows for Team Floor Lead**:
Sets a professional tone and builds relationships that inspire and encourage teammembers Fosters a supportive team environment by addressing conflicts through open, respectful, and constructive communication. Demonstrates leadership by actively engaging in hands-on support and facilitating job shadowing opportunities to help team members enhance existing skills and acquire new ones. Ensures that all team members are equipped with the necessary resources and support to excel in their roles, enabling them to assist individuals in achieving their best quality of life.
Serves as the primary point of contact for families of individuals being supported, addressing their concerns and inquiries. Will escalate matters when necessary.Identifies and reports any unsafe conditions, hazards, practices, accidents, or incidents in compliance with the Occupational Health and Safety Act, while strictly following all Health and Safety guidelines outlined in Post21 policies and procedures.

Post 21 Foundation

**Requirements**:

- Successful completion of post-secondary education (diploma/ certificate) in DSW/Intervenor.
- Strong Knowledge with ASL and a strong proven understanding of communication systems a must.
- Demonstrates a strong understanding and a proactive commitment to learning about the unique traits and requirements of individuals who are Deaf, non-verbal, or have complex care needs, as well as their families.
- Medication oriented and trained. Seamless care trained preferred.
- Must have 1-5 years experience in leadership role. Management abilities.
- Have 5+ years training within the complex/developmental population is preferred.
- Proven professionalism, accountability, responsibility, and reliability
- Aptitude and ability for manual lifting and dexterity
- Work independently and as part of an interdisciplinary team
- Well-developed interpersonal, communication, organizational and problem-solving skills
- Maintains positive relationships to deliver person/family centred care
- Flexible and patient
- Must have valid/current First Aid/CPR level A (minimum)
- Current Criminal Reference Check (for vulnerable population) (within 6 months)
