JOB DESCRIPTION FOR **SOCIAL NAVIGATOR.**

**REPORTING RELATIONSHIP**

This position is responsible to and reports directly to the Operations Manager.

**FUNCTION**
Working closely with the Case Manager, the social navigator will provide and/or coordinate support services to individuals who are experiencing homelessness.

**DUTIES AND RESPONSIBILITIES**
- Facilitate access and entry into social services, health services, employment, and housing
- Facilitate transportation to scheduled appointments
- Ensure clients receive quality services from internal and external providers, in a timely manner. Monitor client outcomes of services, for quality control purposes
- Participate in regular housing meetings, with external organizations
- Ability to make referrals to external services, in collaboration with the clients
- Build and maintain relationships with external service providers.
- Engage clients in volunteer activities
- Facilitate group activities

**QUALIFICATIONS**:

- Degree or college diploma in the social services sector and/or relevant experience is preferred.
- Experience working with clients with mental health and substance use issues
- Ability to work well independently and as part of a team.
- Knowledge and proficiency in current assessment and short-term counseling techniques
- Knowledge and demonstrated ability to work collaboratively with community social service agencies
- Sensitivity to the needs of a diverse community and an understanding of the impacts of social, economic, and cultural issues on marginalized populations
- Ability to deal with crises
- Ability to work within a client-centered, harm reduction approach
- Strong interpersonal, and organizational skills
- Must have a valid driver’s license and access to a vehicle
- Computer literacy in Microsoft Office
- Experience using HIFIS an asset

- Report all health and safety concerns to a representative of the Joint Occupational Health & Safety Committee.
- Take every reasonable precaution to ensure the safety and well-being of staff and clients.

**OTHER DUTIES AS ASSIGNED**

**Job Types**: Full-time, Fixed term contract

**Salary**: $22.50 per hour
