Position Summary
Behavioral Support Consultant
As a Behavioral Support consultant you demonstrate the ability to function independently as well as collaboratively with assigned organizations and team members. You have excellent organizational skills with the flexibility to adjust/adapt work schedules to meet the needs of organizations serviced. You have excellent listening and communication skills. This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.
On a practical level, you will
Provide patient care following current accepted standards of practice and principles of physiotherapy in the assessment, treatment and education of patients, and maintain liaison with other members of the health care team concerning the progress and discharge of patients
Provide educational consultation services to those individuals caring for seniors suffering from behavioural disorders associated with cognitive impairment or other acquired brain injury; seniors with mental illness and significant functional limitations requiring complex care; and vulnerable adults who may be less than 65 years of age but whose cognitive and functional abilities are compromised by developmental disability, Downs Syndrome, and early dementia.
Collaborate with the interdisciplinary team and external partners in the assessment and monitoring of patients with responsive behaviours and the take on a leadership role in the development and execution of individualized behavioural care plans.
Collaborate with the interdisciplinary team in establishing community Behavioural Support linkages to support patients and families as they transition back to the community.
Provide educational consultation service to those caring for frail seniors with cognitive impairment and seniors with other psychogeriatric challenges in acute care.
Liaise closely with behaviour support services in long-term care facilities, community service agencies, and Home and Community Care in order to achieve effective and sustainable discharge planning.
Contribute to continuous quality improvement initiatives within the organizations that are serviced.
Provide leadership in the professional development of those caring for frail seniors with psychogeriatric problems.
Create curriculum materials and prepare reports to meet identified needs.
Supervisor
**Qualifications**:
Undergraduate degree in one of the following health professions: Nursing, Occupational Therapy, Physiotherapy, Psychology, Social Work. Master’s degree preferred.
Membership in good standing with the respective regulatory College.
Minimum of 2 years’ experience in the area of psychogeriatrics preferred; otherwise experience in geriatrics, long-term care, or community-based seniors’ care essential.
Awareness of theory and best practices in adult education.
Knowledge in the consultation process.
Demonstrated competence in the use of standard psychogeriatric assessment tools.
Proficiency in the use of computing software including Word, PowerPoint, Excel, E-mail and the Internet.
Ability to communicate in languages other than English or French is an asset
Experience with bedside coaching.
Familiarity with standardized curricula, such as PIECES, UFirst, GPA, or Montessori, is an asset
