**_ Overview - Comprehensive Care & Integration Specialist Team (CCIS), Community Care_**

The team provides short term case management; attachment to primary care; promotes coordinated care planning within an anti-oppressive, trauma-informed, harm reduction, and mental health recovery framework._

The Health Access Taylor Massey team_**
- The Health Access Taylor Massey team navigators are present on site to provide low barrier access to community health and social services within the Taylor Massey and Oakridge region. The navigators work with a diverse demographic by connecting them to community resources such as primary care physicians, housing supports etc._

What You Will Do_**
- Referrals and Outreach:_
- _ Connect primary care providers to appropriate mental health and addictions supports._
- _ Respond to referrals/requests made to SCOPE by primary care providers._
- _ Support the Health Access Taylor Massey team. _
- _Liaise and develop relationships with service providers/community agencies to make seamless, effective referrals and service connections. _
- _Establish contact with the interprofessional team/community agencies to obtain, provide, or exchange information relating to client’s psycho-social functioning and health care._
- _ Navigate hospital and community resources to assist primary care providers to manage client medical, functional, cognitive, and psychosocial conditions._
- _ Follow-up with clients, caregivers, and/or service providers to communicate recommendations, provide referral status updates, support service navigation and referral to other resources._
- Direct Client Support:_
- _ Perform clinical assessments (psychosocial, psychological, and geriatric) to determine service needs and to formulate recommendations or treatment plans._
- _ Identify appropriate and credible resources responsive to client needs taking into consideration culture, language, reading level, accessibility, and health literacy._
- _ Account for client values, preferences, and circumstances to support the development of goals._
- _ Provide system navigation, service/care coordination, short/long-term planning and follow-up._
- _ Provide supportive/therapeutic counseling to clients utilizing a wide range of approaches that best meet the clients’ needs and circumstances._
- _ Encourage active communication between clients, caregivers, and health care providers._
- _ Provide preliminary assessment of risk in crisis situations, consult, and devise an intervention and action plan, and provide crisis intervention. _
- _Create, maintain, and update appropriate client records, in a timely, efficient manner. _

- _ Collaborate to improve care through transition in health care system/community services._
- _ Enhance work flow and communication pathways between primary care and community agencies by identifying barriers to care and work towards creating new care pathways. _
- _Collect data/outcomes for research and evaluation._
- _ Contribute to the development, improvement, and evaluation of the service when required._
- General:_
- _ Ensure the organization’s policies and procedures are carried out._
- _ Maintain all aspects of confidentiality._
- _ Provide mentorship, training, and/or supervision to student placements and volunteers._
- _ Participate in team/unit meetings and ongoing training and education._
- _ Participate in supervision sessions and annual performance appraisals with supervisor._
- _ Maintain records and documentation in a timely manner and collect statistical data._
- _ Represent the unit on internal/external staff committees as directed._

What You Bring to the Team_**
- _
Masters of Social Work or graduate degree in a relevant field. _
- _Registered Social Worker in good standing with the Ontario College of Social Workers and Social Service Workers or Member in good standing with a Professional College in Ontario._
- _ 3+ years of experience in mental health, addictions, and/or social services, supporting individuals with mental health and substance use issues from an anti-oppressive, anti-racist, harm reduction, trauma informed, and mental health recovery approach._
- _ Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption)._
