Health Sciences North (HSN) is seeking an innovative individual for the position of Dietitian to assess patient’s complex nutrition needs and participate in the development, implementations and evaluation of a comprehensive, interdisciplinary care plan. The position will be specifically in the Diabetes Education program. The focus of the role will be providing individual and group appointments to patients living with diabetes and/or gestational diabetes within an interdisciplinary team. The team includes Nurses, Social Worker, Pharmacist, Chiropodists, Physiotherapists, Nurse Practitioners, Endocrinologists and other Dietitians.

The Community Care and Rehabilitation Program is a multi-faceted program focused on attaining the highest quality of life for individuals by maximizing abilities, maintaining or improving independence and assisting persons to compensate for functional limitations.
