**REPORTS TO**: Interdisciplinary Team (Den) Manager
**Job Purpose**:

Reporting to Interdisciplinary Team Manager, the Adult Case Manager works within the Interdisciplinary Team Den and is responsible for the day-to-day care of community members in their den with emphasis on mature, adult issues. This includes trial entry into the den, through to goal planning, graduation, and post-graduation.

**Position Responsibilities**
- Assist the Interdisciplinary Team Manager, in the daily operations of the den by working closely with community members in meeting their day-to-day needs.
- Support the creation of an inclusive, safe and friendly environment for all staff, community members and external parties.
- Build rapport with community members by identifying strengths and working in a respectful way with a demographic who has faced trauma and social exclusion.
- Maintain accurate and comprehensive written documentation of all den community members related interactions in accordance with den standards.
- Maintain a strong knowledge of programs offered within the den and throughout the organization to be able to refer community members.
- Refer community members to community-based services and advocate on their behalf when needed in an appropriate manner.
- Deliver crisis intervention as needed, including risk assessment and follow-up until the situation has stabilized.
- Organize and facilitate case planning meetings, inclusive of entire den and community members.
- Maintain a professional and welcoming workspace and den
- Attend team and organizational meetings as required.
- Participate in required trainings and professional development.
- Complete any administrative duties as required.
- BSCS is more than a workplace. It is a community where everyone helps and contributes to the whole, and there is an expectation that all employees will take part in events (during the work week and occasionally during weekends and evenings). This includes fundraising campaigns, running community meals, medicine picking etc.
**Qualifications**
- Class 5-non GDL driver’s license, access to a reliable personal vehicle with $2,000,000 liability insurance, and a clean driver’s abstract
- Post-secondary Degree in a relevant field of study
- Minimum 6 months’ experience working with the vulnerable population
- Ability to respond appropriately in stressful situations and engage in de-escalation tactics when conflict arises
- Excellent knowledge and understanding of poverty related issues, trends, and services particularly, but not limited to the urban aboriginal population is essential
- Previous experience with crisis-intervention preferred
- Valid First Aid certificate or enrolled in a future course
- Police Check with the Vulnerable Sector completed within the past six months and kept up to date (every 3 years)
- Child and Youth Intervention Check completed within the past six months and kept up to date (every 3 years)
- Experience with and knowledge of Indigenous culture and ceremonies is considered an asset.
