Epilepsy System Navigator

Epilepsy South Eastern Ontario (ESEO) is a not for profit charitable corporation dedicated to improving the quality of life for those affected by epilepsy and seizure disorders. We strive to provide education, advocacy, and support services for those affected by epilepsy and seizure disorders.

**Position Summary**:
We are seeking a compassionate and dedicated Epilepsy System Navigator to join our team. This part-time role involves supporting clients primarily in pediatrics (children and youth) and their families affected by epilepsy and seizure disorders, helping to navigate services in the medical, education, early intervention, and mental health service systems, counselling, completing forms, assistance with funding packages, advocacy, and sourcing community-based services and resources.

**Key Responsibilities**:
Provide individual and group counselling support to clients and families, helping them to understand and manage epilepsy.

Support the District Epilepsy Clinics at Kingston Health Science Centre by providing educational information, face-to-face counselling, the development of seizure response strategies for families and other related educational opportunities. Attend in-person clinics.

Assist in navigating the healthcare system, including medical, social, and community resources.

Assist in transitioning from pediatric to adult care epilepsy treatment.

Present and provide educational sessions on epilepsy management, seizure first aid, and related topics.

Advocate for patients' needs within education, the healthcare system and community settings.

Maintain accurate and confidential records of client interactions and support provided.

Case management and planning to address a clients or family/caregiver needs.

Collaborate with healthcare professionals, community organizations, and support networks to ensure comprehensive care.

Stay updated on the latest research, treatments, and resources related to epilepsy.

**Qualifications**:

- College diploma or bachelor’s degree in health sciences, social work, or a related field preferred.
- Experience in a healthcare or community support role, particularly with chronic conditions.
- Highly organized with outstanding written and verbal communication skills.
- Advanced skills with Microsoft office (Outlook, Word, Excel, PowerPoint), Intranet (SharePoint), audio-visual technology and internet knowledge.
- Proficiency in using digital tools and databases for record-keeping and resource management.
- Demonstrated understanding of data collection methods and the ability to gather and analyze data for outcome measurements effectively.
- Ability to handle sensitive information and maintain confidentiality
- Demonstrated team player with professional approach in a team-based environment
- Strong interpersonal skills that enhance teamwork within a small organization

- Empathy and the ability to provide non-judgmental support to clients.
- Ability to work independently and as part of a multidisciplinary team.
- Valid driver's license and vehicle. Travel is required for this position.
