**Background**:The MINT Memory Clinic offers interprofessional, primary care-based assessment, early diagnosis, and ongoing support for individuals with cognitive impairment and dementia. Supported by a team of specially trained healthcare professionals and community partners including Alzheimer Society of Peterborough, Kawartha Lakes, Northumberland and Haliburton, the Northumberland Family Health Team, and Seniors Care Network, the clinic collaborates closely with primary care providers, geriatric specialists, and other health and social service agencies to deliver compassionate, person-centered care to clients and their families./care partners.

**Job Summary**:This role combines clinical and administrative responsibilities to support the delivery of comprehensive memory care services. Clinically, the RPN performs cognitive testing, geriatric assessments, and participates in interprofessional care planning for clients and their families/care partners. Drawing on specialized knowledge in dementia and cognitive impairment, the RPN supports diagnosis, develops and implements interventions, and provides consultative support for responsive behaviours.

Administratively, the role provides essential clerical support to the interdisciplinary memory clinic team. Responsibilities include managing referrals, gathering background information, booking appointments and clinic space, coordinating clinic schedules, and supporting communications. The role also offers general program support to organizational members of the clinic. This integrated position ensures both the clinical and operational aspects of the memory clinic run efficiently and effectively.

**Service Responsibilities**:
**Administrative Support**
- Receive and process referrals, complete client registration, and gather required documentation for triage and clinic days.
- Schedule client and collateral appointments, provide reminders, and manage booking of clinic space and resources.
- Prepare documentation and clinic materials; draft correspondence, referral letters, and care plan communications.
- Maintain accurate filing systems, confidential records, and clinic data.
- Support clinic operations by attending scheduled assessment days, welcoming clients/caregivers, and providing on-site administrative assistance.
- Assist with team meetings (e.g., agendas, scheduling, minutes), develop service delivery templates and promotional materials, and participate in committee work.
- Provide cross-coverage for other CHCN administrative staff as needed.

**Clinical Support (RPN Scope)**
- Conduct cognitive and functional assessments, behavioural evaluations, and geriatric interviews as part of the interprofessional assessment team.
- Participate in care planning and follow-up, including communication with families, third parties, and health system partners to support coordinated care.
- Collaborate with clients, families/care partners, and interprofessional team members to identify goals and create person-centered care plans.
- Attend MINT clinical assessment training and ongoing refresher education.

**Required Knowledge, Skills, and Abilities**
- Strong knowledge of dementia, delirium, depression, and cognitive/mental health conditions, including assessment tools and best practices.
- Understanding of responsive behaviours, behavioural expression, and strategies for dementia care.
- Proficiency in therapeutic and pharmacological approaches to behaviour management.
- Excellent assessment, critical thinking, and clinical judgment skills within the full RPN scope of practice.
- Effective interpersonal and communication skills; ability to foster collaboration within an interprofessional, community-based care model.
- Demonstrated organizational skills, professional work ethic, and ability to prioritize in a fast-paced, client-focused environment.
- Proficiency in clinical documentation software, telemedicine, virtual platforms, and standard office tools.
- Commitment to quality improvement, knowledge exchange, and capacity building within the team and broader system.
