**Full-Time**

At Community Care Durham (CCD), coordinated networks of volunteers and inter-professional staff, working out of integrated community hubs and other locations, strive to enrich the well-being and quality of life for people wherever they choose to call home. As the largest community support services agency in Durham Region, CCD collaborates with its partners and the broader community to address community issues and support local citizens to achieve their goals to live independently.

Reporting to the Director of Community Supports Services, the Manager of Community Wellness and Access provides leadership and oversight for key community programs, including WeConnect Mental Health & Wellness, Exercise and Falls Prevention, Transportation, and Visiting & Social Safety. This role monitors program performance ensures compliance with organizational standards and contributes to continuous quality improvement for effective service delivery.

Responsibilities include but not limited to:

- Serves as a key change leader, advancing service improvements in alignment with CCD’s Strategic Plan and directions; 
- Develops and implements process documents, operational guidelines, and best practices to support high quality client services; 
- Provides oversight and leadership to the following programs: WeConnect, Exercise and Falls Prevention, Transportation, Telephone Reassurance, and Friendly Visiting;
- Monitors program outcomes against MSAA targets to manage program performance;
- Oversees the preparation and submission of monthly program reports and performance metrics;
- Provides leadership and guidance to support the safe, effective, and client-centered delivery of services, with a focus on continuous quality improvement;
- Ensures program operations remain compliant with applicable legislation, regulatory requirements, and internal policies;
- Evaluates client services and program operations to identify opportunities for improvement and innovation that align with the organization’s Strategic Plan and responds to evolving community needs;
- Conducts performance evaluations for direct reports and provides guidance through coaching, mentoring and recognition strategies; 
- Supports the implementation of effective volunteer management practices, including training and education;
- Manages program budgets, ensuring financial accountability and sustainability;
- Builds and maintains strong relationships with community organizations, stakeholders, and service providers to enhance collaboration and resource sharing;
**Qualifications required**:

- Post-secondary education in a relevant field such as Social Service Work (SSW), Psychology, Health Studies, or related field is required
- A minimum of three (3) years of experience in a supervisory or management role is required
- At least two (2) years of experience in the community mental health sector
- Experience within a community-based not-for-profit organization is an asset
- Strong proficiency in Microsoft Office (Word, Excel, Share Point, etc.) is required
- Understanding of mental health and privacy related legislation and policy, including the Mental Health Act and Personal Health Information Protection Act (PHIPA)
- Knowledge of volunteer engagement principles
- Proven ability to provide leadership, sound judgment, and critical analysis in high-pressure or complex situations
- Exceptional communication and interpersonal skills
- Experience with budget development, financial oversight, and resource planning
- Skilled in managing multiple priorities and balancing competing demands effectively
- Demonstrated strategic planning abilities and advanced critical thinking skills
- Possession of a valid Ontario “G” driver’s license
- Sensitivity to the needs of the vulnerable population with disabilities, including challenges in cognition  
- Satisfactory Police Vulnerable Sector Check  
- Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

Compensation: Starting at $75,000 annually; 3 weeks’ vacation per year of work (pro-rated to hours worked); 9 paid (including stat) holidays, 3 paid float days; health, insurance and pension benefits when eligible.
- Pay will be commensurate based on education and experience
