**Overview**:
The Clinical Dietitian, as a member of the Health Care Team, identifies, develops, plans, implements, and evaluates all nutrition interventions and education requirements for patients and members of the Health Care Team.
As a member of the Diabetes Education Team, the Registered Dietitian provides nutritional counselling services and participates in the education and support of the adult with diabetes and his/her family. The Registered Dietitian provides follow up visits for diabetes self-care based on individual needs in individual, team, group or clinic settings. As a member of the Diabetes Education team, and in collaboration with our community partners, the Registered Dietitian plans, develops, implements and evaluates outpatient diabetes education services.
**Qualifications**:
- Honours Baccalaureate Degree in Human Nutrition.
- Completion of an Internship or combined Masters/Internship program from a Dietitians of Canada accredited facility.
- Member in good standing with the College of Dietitians of Ontario
- Membership with Dietitians of Canada
- Certified Diabetes Educator (CDE)
**Required Skills**:
- Minimum 2-5 years recent/related experience in an acute care setting, involving both inpatient and outpatient nutrition care including: nutrition support, surgery, medicine, rehabilitation, complex continuing care, tube feeding, TPN, diabetes mellitus management, renal management, and gastrointestinal disorders is preferred.
- Demonstrated excellent interpersonal, verbal and written communication skills.
- Ability to communicate effectively with all members of the Health Care Team, at all levels.
- Awareness of community resources for the person/family with diabetes
- Knowledge of current standards and guidelines for the management and education of people with diabetes
- Knowledge and experience in calculating TPN and tube feeding
- Advanced diabetes education and management skills including insulin-to-carbohydrate ratio, intensive insulin therapy and use of medical directives within the diabetes care team
- Ability to initiate, develop, implement and monitor program objectives based on current clinical practice guidelines and best practices
- Demonstrated leadership skills.
- Demonstrated educator skills.

- Innovation with delegation, decision making and problem solving skills.
- Team player.
- Computer skills, including use of Microsoft Outlook, PowerPoint and Meditech system
