Client Care Manager

We are looking for a passionate new team member to join our growing company. This person will hold a large knowledge of the healthcare industry and enjoy the flexibility a small businesses has to offer.
**Roles and Responsibilities Client Care**
- Establish and monitor a care client plans to ensure the level and quality of care is met
- Ensure progress notes are complete and maintained in an orderly fashion.
- Ensure client documentation is complete as per guidelines.
- Discuss client needs with families, physicians, and other health care professionals.
- Liaise with families to ensure agreements and on-boarding documents are signed prior to service.
- Demonstrate the ability to interact effectively within the team, with residents and clients and families as well as external service resources.
**Qualifications and Education Requirements**
- RPN/RN Diploma/Certificate is preferred
- College/University degree in healthcare is an asset
- 2/3 years job related experience as a Scheduler/Coordinator in a healthcare setting
- Efficient verbal and written communication skills
- Ability to multi - task and problem - solve independently in a fast - paced environment
- When off duty, be prepared to consult or give advice over the telephone and/or be on call for emergencies.
- Work hours may vary with some evening and weekend work
**Job Types**: Full-time, Part-time
Part-time hours: 24-40 per week
**Salary**: $40,000.00-$60,000.00 per year
**Benefits**:
- Extended health care
Schedule:
- Monday to Friday
- On call
- Weekends as needed
Supplemental pay types:
- Bonus pay
- Overtime pay
**Experience**:
- Nurse Managers & Supervisors: 1 year (required)
Licence/Certification:
- RPN/ RN in Ontario (required)
Work Location: Hybrid remote in Oakville, ON
