Clinical Nurse Specialist

Reporting to the Manager, Nursing Practice and Education, the CNS Wound Care demonstrates advanced knowledge, skill and judgment in providing comprehensive specialized care to patients that is consistent with the College of Nurses of Ontario standards.The CNS Wound Care requires strong leadership and communication skills and advanced nursing knowledge and skills to support patients, families, and the interprofessional team. The CNS Wound Care works collaboratively with other members of the nursing practice team and interprofessional team when providing care and to support the implementation of key nursing and interprofessional initiatives including the implementation of best practices to optimize patient outcomes and enhancing safe quality care. At Unity Health Toronto (UHT), the CNS Wound Care must be able to demonstrate the ability to:support the delivery of quality care to diverse patient populations, with complex wounds and ostomies, and facilitate the development of comprehensive care plans in conjunction with the inter professional team to enhance patient safety, outcomes, access to care, and experienceblend clinical expertise with education, knowledge translation, leadership and research skills to serve asa consultant and resource to nurses, nursing leadership, inter professional teams and external partners in the process of decision-making, developing clinical guidelines and protocols, promoting the use of evidence, and facilitating system change.RESPONSIBILITIES:The CNS works within 4 domains of practice.Clinical DomainInvolved in direct and indirect patient care using a systems-oriented and population-focused approachSupports the holistic nursing management of complex wounds and ostomiesCollaborates with the inter professional team in the development of comprehensive care plans for complex or high risk patientsActs as patient advocate and partners with patients and familiesCollaborates with internal and external partners for the transition and coordination of patient care, including discharge planningKnowledge TranslationCritically appraises research and applies it to practice, quality and patient safety initiativesBuilds nursing capacity by supportingevidence based practice at the point of careMentors nursing students (undergraduate and graduate) and staff, e.g., professional development and quality improvement initiativesLeads and collaborates in the development of education programs based on current evidence for patients/families, nurses and inter professional team
