**Job Summary**
We are seeking a compassionate and dedicated Health Care Aide to join our team. In this role, you will provide essential support to patients in various settings, including assisted living facilities, nursing homes, and hospice care environments. Your primary responsibility will be to assist with daily living activities and ensure the comfort and well-being of our patients. This position requires a strong commitment to patient care and the ability to work collaboratively with healthcare professionals.

**Duties**
- Assist patients with daily living activities such as bathing, dressing, grooming, and eating.
- Administer medications as prescribed and monitor patients for any side effects or changes in condition.
- Provide companionship and emotional support to patients, fostering a positive environment.
- Assist with mobility and transfers, including heavy lifting when necessary.
- Maintain cleanliness and organization in patient areas, ensuring a safe environment.
- Document patient care activities accurately and report any changes in health status to nursing staff.
- Support patients with dementia care by using appropriate communication techniques and activities tailored to their needs.
- Collaborate with nurses and other healthcare professionals to create individualized care plans.

**Skills**
- Experience in assisted living or senior care environments is preferred.
- Knowledge of medication administration protocols is essential.
- Strong caregiving skills with a focus on patient-centered care.
- Ability to perform heavy lifting tasks safely and effectively.
- Familiarity with hospice care practices is an asset.
- Excellent communication skills for interacting with patients, families, and healthcare teams.
- Compassionate demeanor with a genuine desire to help others improve their quality of life.
- Ability to work effectively in a fast-paced environment while maintaining attention to detail.
