**This is a 12-month maternity leave position**, with the potential to lead to a permanent position pending an assessment of business need.

As a Care Manager you are responsible for the day-to-day operations related to supporting business results through client experience and caregiver management. You will drive growth through the following main duties and responsibilities:
**Key Activities and Responsibilities**:
Client Care Manager
- Conducts client assessments to identify health care and home support needs, resulting in the development of a comprehensive and individualized care plan for each client.
- Provides coaching to client care team to ensure care plans are executed effectively and to the Promyse standard of care
- Provides delegations and in-service training as needed
- Provides ongoing client oversight and updates to care plan as needed
- Conducts supervisory visits to ensure quality of care and client satisfaction with services
- Provides nursing care and consultation, as required
- Collaborate with other health professionals and client circle of care to ensure optimal client outcomes

Community Liaison
- Development of relationships with community partners and health care professionals
- Raising awareness of Promyse Home Care with Health Care Providers
- Strong communication, leadership, management, and training skills
- Critical thinker and problem solver, detailed and organized.
- Able to manage time, workload and conflicting priorities effectively and efficiently
- Assessment and case management experience
- Strong organizational skills
- Flexibility in work schedule
- Proficiency in Windows-based software programs

**Position Requirements**:

- Current nursing registration in good standing with the provincial regulatory body
- Valid CPR certificate
- Vulnerable Sector police check
- Negative TB test
- Ability to speak, read and write fluently in English
- Vehicle for business use and valid driver’s license

**Required experience**:
Nursing: 5 years
