One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.
**QUALIFICATIONS**:
- Bachelor’s degree in Social Work (HBSW), or equivalent relevant experience.
- Ability to effectively engage clients in assessment of needs and goal planning.
- Knowledge of and connections with community services and partners, including but not limited to health and social services, the judicial system and Indigenous organizations.
- Demonstrated ability to facilitate and advocate for services to support clients.
- Knowledge and background in the areas of trauma-informed care, culturally safer care, and working with individuals who use substances.
- Sensitivity to the needs of a multicultural community and the impacts of social, economic and cultural issues on health and well-being.
- A current driver's license and access to a vehicle to travel to designated outreach sites.
