Community Ambassador

**Carefirst Seniors & Community Services Association** is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area. With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail. We are currently seeking a **Community Ambassador **to join our Locally Driven Population Health Models (LDPHM) Program. **Terms**: Full-time Contract (1 year) **Responsibilities**: Working closely with an inter-professional team, the Community Ambassador will play an integral role of: - Engaging in culturally responsive engagement and outreach to individual community members and provide accurate and relevant information/education about the importance of preventive health care, cancer screening, vaccination, chronic disease prevention and management, mental health promotions, access to primary care, and other identified local population health challenges - Coordinate and conduct culturally and linguistically responsive educational presentations individually and within groups in the communities - Engage community members in preventive care conversations and health action planning - Connect community members to the right services and programs that support the determinants of health - Conduct grassroot community outreach activities, i.e. in-person and virtual/social media - Liaise with diverse local partners to address local population health priorities - Participate in relevant committees and service networks internally/externally - Collect and track and help analyze program statistics for trending and reporting and program evaluations - Administrative duties to enable the optimal function of a collaborative team - Other duties and responsibilities may be assigned from time to time - Report to Director overseeing the Locally Driven Population Health Models (LDHM) **Qualifications**: - Post-secondary education (Degree/Diploma) in health, health promotion and human services - Experience in community services is an asset - Excellent communication and presentation skills to clearly convey information in a manner that engages the audience and helps them understand and retain messaging. - Must be able to communicate verbally and in writing in English and a second language that reflects the communities that we serve (e.g. Tamil, Urdu, Punjabi, Hindi, Cantonese or Mandarin, etc) - Must be able to work a variety of shifts including evenings and weekends and from a variety of locations within the York Region and City of Toronto - Demonstrate interest and understanding of equity, diversity, anti-racism, and inclusion framework, population health approaches. - Demonstrate a knowledge of communities and neighbourhoods in York Region and surrounding areas - Strong interpersonal, communication, conflict resolution and time management abilities - Self-directed, self-motivated, with a positive attitude and be able to work independently and as part of a team. - Ability to prioritize, multi-task and adjust to shifting priorities. - Valid Ontario driver’s license and access to a reliable vehicle - Current vulnerable sector check (Completed within one year). **Benefits**: - Comprehensive Orientation Program - Training and Education Subsidy **Application Deadline: 22-12-2024** - Carefirst is an equal opportunity employer.
