Job Description:
**Job Summary**:
The Dual Diagnosis Systems Specialist provides enhanced case management and service navigation for youth with co-occurring developmental disabilities and mental health needs. This role supports families in accessing appropriate services, coordinates care across systems and strengthens collaboration and capacity within the Children’s Mental Health and Developmental Services sectors.

This position is covered by the Ontario Public Service Employees Union on behalf of its local 213.

**Duties & Responsibilities**:
**Case**_Management_**

- Conduct comprehensive intake assessments to identify the needs, strengths, and risk factors of youth with co-occurring developmental disabilities and mental health concerns.
- Develop individualized service and support plans in collaboration with the youth, their family or caregivers, and relevant community partners.
- Coordinate multidisciplinary input, including psychiatric consultation, psychological assessment, and specialized clinical services as needed.
- Facilitate case conferences and team meetings with internal and external stakeholders

**_Service Navigation and System Integration_**

- Act as a liaison between Children’s Mental Health (CMH) and Developmental Services (DS) sectors, ensuring collaborative communication and coordination among agencies, schools, and medical providers.
- Identify and reduce barriers to accessing appropriate services and supports within community systems.
- Support families and youth in understanding available service options, eligibility criteria, and referral processes.
- Build and maintain strong partnerships with local CMH agencies, developmental service providers, educational systems, and community health organizations.
- Participate in regional planning initiatives or interagency committees to strengthen cross-sector collaboration and address service gaps

**_Administrative duties_**

- Maintain accurate, timely, and confidential client records in accordance with organizational policies, privacy legislation, and professional standards.
- Document assessments, service plans, progress notes, and case reviews clearly and comprehensively.
- Manage program waitlists, ensuring triage based on acuity, risk, and service priorities.

**Qualifications**:

- Post-secondary education in Social Work, Autism and Behavioural Science, Psychology, Sociology, or a related human services field.**
- A College Diploma in Social Service Work or Developmental Services Work may be considered.**
- Registration or eligibility for registration with a regulated health profession is considered an asset.
- Relevant practical experience in case management, developmental services, or children’s mental health is required.
- Must possess a valid Ontario driver’s license, reliable vehicle, and automobile insurance including business use coverage with a minimum liability of $1 million.
- Ability to travel throughout the Peel and Halton regions as required.
- Bilingualism (English/French) and proficiency in other languages are considered assets.
