Reporting to the Program Supervisor and Manager of Participant Services, the Program Coordinator (PC) is a full-time coordinator position responsible for providing direct support to
Metro Community Living’s (MCL) participant population in collaboration with MCL’s front line Community Residential Workers (CRW) and the Programming Team. The PC is also responsible for the daily planning, organization and supervision of the Company’s programs, service delivery and front-line staff.
Regular PC working hours are 8:30am-4:30pm. However, the hours of work for the PC position are flexible and can be unpredictable in order to respond to participant and programming needs.
**Salary range**: $51,000-$54,000
**Duties**:
**_Participant Planning and Support_**
- The PC is responsible for providing direct participant support as required.
- The PC will provide direct participant support that is required to complete needs and risk assessments in order to develop and fulfill the Individual Program Plan (IPP)
- Develops, implements and makes revisions to participant care plans and programs (IPP’s) in consultation with the participant, Program Supervisor, family, front-line staff and other health care professionals.
- The PC will conduct formal and informal assessments in an effort to understand the participants stated wishes, daily living skills, environmental resources, motivations, latent capabilities and social networks, which are important in developing a comprehensive understanding of the client. This will require repeated personal contact and observation, client interviews, goal plans, quarterly reports, medical and psychiatric records and information from relevant others.
- The PC will work collaboratively with the participants treatment team and foster positive relationships with community partners.
- Provides leadership to staff and management in the planning and implementation of extra staffing (ES) hours. Ensures the budgeted ES hours are utilized effectively.
- Ensures program goals and objectives are developed collaboratively with participants, staff and the Program Supervisor.
- Ensures required participant reports are completed.
- Ensures participants attendance at all medical, psychiatric and therapeutic appointments and that treatment recommendations are implemented.
- Ensures participant Special Needs Requests are identified and submitted to the Program Supervisor in a timely manner.
- Identifies and documents any participant and family concerns, informs the Program Supervisor of all participant and family concerns, and ensures that all action plans and necessary follow up are completed and documented in a timely manner.
- Determines the level of intervention and/or support required in the event of a crisis situation or incident, and take measures in an attempt to ensure the appropriate level of support is provided to the participant. The PC will develop and implement emergency response protocols for the participants as is appropriate and necessary.
- Participates in the participant intake process and discharge planning.
**_ Operations_**
- Ensures the residential environments are in compliance with provincial licensing standards and address deficiencies as required. The PC will ensure that participants residential environments are clean and tidy. In addition, the PC will report the need for property repair and /or attention to the Program Supervisor (or designate.) The PC will ensure the required fire and life safety checks are completed.
- Ensures compliance with additional regulatory requirements that are typically not addressed by provincial licensing standards, such as fire and safety regulations.
- Ensures the effective and efficient use of all participant resources (i.e. electricity, heat, water, phone, computer, etc.)
- Monitors household inventories and office supplies.
- Participates in Emergency on Call (EOC) schedule.
- Works collaboratively with the Program Supervisor (or designate) to develop new policies, procedures and protocols as required.
- Submits monthly participant cash requests.
- Ensures MCL receipts for billable items are provided to Administrator in a timely manner.
- Reviews household communication book and addresses all concerns in collaboration with the Program Supervisor.
- Ensures proper documentation is being completed by staff.
- Completes incident reports as required and forwards a copy to the Program Supervisor. Reviews incident reports and forwards to Program Supervisor for final review. Ensures a copy of the incident report is kept in the incident report binder.
- Performs other related duties as assigned by the Program Supervisor.
**_Human Resource Management _**
- Ensures orientation of new employees.
- In collaboration with the Program Supervisor, provides leadership, direction, motivation and guidance to staff.
