**Position Type**: Full Time 37.5 hours per week (flexible schedule with some evening and weekend work)

Position Term: August 1, 2025 - January 7, 2026 with possibility of extension

**Location**: Primarily Vancouver with some work within the Lower Mainland area, BC

**Compensation**:From$29.17 per hour - as per Community Living Collective Agreement. + health and dental benefits upon successful completion of your probationary period (3 months).

- pos_**Abilities** provides a broad range of services to persons with developmental disabilities and their families. We currently serve more than 1,200 children and adults in Metro Vancouver and other parts of BC. **About the Position**:
Journey Facilitators work alongside individuals through a deep-dive discovery that explores identity, motivations, gifts, and goals. To offer guidance to individuals in navigating which of the outreach services offered by _pos_**Abilities** is the right fit for their future goals. To provide ongoing case management throughout individual service journey.

- Well-developed interpersonal and verbal/written communication skills
- Ability to work independently and as part of a team
- Commitment to the Association’s Code of Ethics, Values and Mission
- Working knowledge of information technology
- Strong administrative skills
- Good organization and time management skills
- Knowledge of the needs of youth in transition and adults with developmental disabilities
- Familiarity of Association and local community resources and services

**You MUST have the following**:

- Diploma in Social Work, Social Sciences, Human Services or an equivalent combination of education and experience
- Minimum two years of experience working in a highly collaborative environment and with multi-stakeholder groups
- Demonstrated ability to work with individuals with diverse and unique communication styles
- Class 5 driver’s license, acceptable driving record
- Reliable car as required and $3,000,000 insurance liability
- Current Emergency 1st Aid and CPR
- Current TB test
- Criminal Record Check for Children and Vulnerable Adults - clearance

**YOUR** **Typical Responsibilities**:

- Facilitates a service journey experience from a person-centered approach, with an emphasis on self-determination, identity, values, and individuality.
- Works alongside participants to inspire and motivate them throughout a facilitated self-exploration, contributing to the co-design of their action plans outlining their goals, especially their ability to choose fuller and meaningful lives.
- Uses discovery tools to initiate conversations and understanding around service options, providing the participant with information to make informed decisions about the service journey.
- Builds the competencies and confidence of participants to self-initiate around the journey inward, including spirituality, philosophy and identity, and to reflect on the sort of life they want to live.
- Implements the strategies, principles and frameworks for Explore within the Association.
- Provides information on all services offered by the Association to participants and other stakeholders.
- Coordinates, designs, monitors and plans service journeys based on desired individual outcomes with practice support from the Journey Coordinator.
- Responsible for fostering and coordinating communication with participants, their networks, stakeholders, and other Association services. This involves convening regularly throughout participants’ service journeys, including roundtables, introductory meetings, and ongoing updates.
- Seeks and mobilizes resources/partnerships/community assets to support the needs of individuals throughout their Explore journey.
- Collaborates with participants, their networks, and other Association services to address any issues that arise during their service journey with support from the Journey Coordinator.
- Monitors, adapts and troubleshoots service journeys with practice support from the Journey Coordinator.
- Liaises and works alongside professionals & staff who are involved in individuals’ service journeys.
- Inputs data in a timely manner and ensures that it is accurate and current including participant information on the database.
- Completes all required documentation and reporting.
- Participates in meetings with other stakeholders with direction from the Journey Coordinator.
- Manages a caseload and monitors the number of participants including the intensity of support known for each participant. Informs the Journey Coordinator when there is capacity for additional referrals.
