At HepNS, we are dedicated to supporting individuals at risk of contracting hepatitis, including, but not limited to, people who use drugs, those who are incarcerated or recently released, and people who are insecurely housed or unhoused. **Job Summary**

As an Outreach Worker at HepNS, you’ll be on the front lines of our mission, engaging with individuals and communities to connect them with essential resources and support services. This role involves building relationships, assessing needs, providing information, and linking clients to appropriate resources. You’ll collaborate with a dedicated team to make a meaningful impact in the community.

**Key Responsibilities**
- Conduct outreach activities, including on-site visits and community engagement efforts, to identify individuals who may benefit from our services.
- Build and maintain strong relationships with clients, acting as a trusted resource and advocate for their needs.
- Assess client needs and refer them to relevant services, including housing assistance, healthcare, and harm reduction programs.
- Collaborate with community organizations, health providers, and social services to create a network of resources.
- Participate in events, workshops, and presentations to educate the public on available resources and services.
- Keep accurate, detailed records of interactions with clients and outcomes of services provided.
- Maintain up-to-date knowledge of community resources and maintain a strong working relationship with partnering agencies.

**Qualifications**
- **Experience**: Prior experience in outreach or working directly with underserved populations is a plus.
- **Lived Experience**: Lived experience with drug use, hepatitis, incarceration, or homelessness is highly valued for this role, as it enhances the ability to connect and empathize with our client base. If you're comfortable, please disclose your lived experience in your cover letter/resume.

- Knowledge of community resources and social services (or willingness to learn)
