Palliative Pain and Symptom Management Consultant

Company Bio Home and Community Care Support Services South East is one of 14 Home and Community Care Support Services organizations in Ontario with a focused mandate to deliver local health care services such as home and community care and long-term care home placement. These organizations were previously known as Local Health Integration Networks (or "LHINs") at a time when they also led local health care planning and funding, and those functions are now part of Ontario Health. Home and Community Care Support Services South East is dedicated to ensuring the ongoing delivery of local services while Ontario makes changes to improve the health care system to give patients better-connected care with health care providers working as one coordinated team in Ontario Health Teams. The south east region extends from Brighton on the west, to Prescott and Cardinal on the east, north to Perth and Smiths Falls, and back to Bancroft. The boundaries are for funding and planning purposes only. Residents of the Home and Community Care Support Services South East can seek health care services wherever they prefer, inside or outside these boundaries. Position Summary Deliver consultation, case-based education, and mentorship to service providers of palliative care in a variety of care settings throughout South Eastern Ontario. Collaborate with and coordinate resource teams and community providers in the provision of high quality, evidence-based palliative care services. **Responsibilities/Accountabilities**: - Link providers with specialized hospice palliative care resources; - assessment tools and best practice guidelines; - Provide case-based education and mentoring for service providers incorporating principles of adult learning; - Collaborate with community partners in educational initiatives; - Help build capacity amongst front line service providers in the delivery of palliative care; - Participate on relevant committees and task forces of Home and Community Care Support Services South East and/or South East Regional Palliative Care Network; - Contribute to data collection, quality improvement programs and program evaluation; - Contribute to the development of policies and procedures; - Identifies and participates in ongoing professional development and staff education; - Performs other duties that may be assigned. Minimum Qualifications & Position Requirements - Holds current unrestricted registration with the College of Nurses of Ontario as a Registered Nurse - Minimum three years’ experience in palliative care in a variety of care settings; - CNA Certification in Hospice Palliative Care Nursing (or equivalent); or working towards - Front line nursing experience providing a palliative care approach essential; - Hold current CPR- Level C & AED certification - Vehicle and valid Ontario driver’s license is required Competencies - Highly developed communication, organization, coordination and time management skills; - Demonstrated knowledge of nurse’s role in palliative care and commitment to interdisciplinary care; - Demonstrated skills in problem solving and critical thinking; - Excellent interpersonal and teamwork skills; - Demonstrated commitment to continuous learning and professional development; - Computer proficiency; - Demonstrated knowledge and expertise in mentorship and case-based education; - Knowledge of community and LTC sectors Closing Statement Home and Community Care Support Services has implemented a mandatory vaccination policy across the province that requires all staff to be fully vaccinated against COVID-19.
