Metis Patient Support Worker

**Brief Overview**:
The Métis Nation-Saskatchewan is the government that represents Métis citizens in
Saskatchewan, and MN-S Ministry of Health is responsible for improving the health and well-being of Métis citizens in Saskatchewan. The Ministry of Health does this through program development and operation, research and data partnerships, and the development of a Métis Health and Well-being Strategy. MN-S Ministry of Health requires five Métis Patient Navigators to work independently and collaboratively with MN-S Ministry of Health to develop and operate a Métis-specific Patient Navigation Program for Métis citizens in Saskatchewan. The goal of this program is to support and facilitate safe and accessible primary care and community-based health care service for Métis citizens. Key partnerships are with the Saskatchewan Health Authority First Nations and Métis Health Services Branch, SHA health care providers in North Battleford, Saskatoon, Prince Albert and Regina, and other MN-S Ministries.
**Key Responsibilities**:
- Build trusting relationships with citizens and medical professionals
- Liaise with citizens and primary care teams who provide direct client and family support, to ensure their successful access to primary care and community-based health care services
- Be responsible for ensuring that patient care occurs in a culturally safe and respectful manner by:
- connecting with clients and families to offer support,
- helping clients and families voice their questions or concerns to the primary care teams,
- offering traditional culturally safe resources that compliment current healthcare regimes,
- mitigate potential complaints by acting as a conduit between client and care team through presence, communication, approach, and understanding.
- Liaise with a variety of Saskatchewan Indigenous community-based agencies
- Inform and influence program development, planning and evaluation of services for Métis citizens through the patient experience process of navigating the healthcare system
- Support and attend community engagements hosted by MN-S health team
- Support recruitment of Elders and citizens to interview
**Qualifications **- Bachelor’s degree in health care, nursing, social work, or related discipline plus completion of 3-5 year’s work experience or an equivalent combination of education and experience with Métis peoples in Saskatchewan**Knowledge and skills**:
- Knowledge of Métis culture
- Strong communication skills including speaking, writing, and active listening skills
- Strong computer skills (including Microsoft Word, Excel, database software, electronic communication tools such as Microsoft Outlook)
- Strong time-management skills along with the ability to prioritize daily tasks, organizational skills, and effective decision-making abilities
- Willingness to work within a team environment
- Professional telephone etiquette
- Skills in general administrative functions, including accurate typing, filing, ordering office supplies, maintaining office supplies and equipment
- Exceptional written and oral communications skills
- Excellent work attendance
- Sound problem solving and decision-making skills as well as attention to detail
- Interpersonal skills and an ability to demonstrate tact and diplomacy in difficult situations
- Strong awareness of current Métis health issues and the current context of health governance in Saskatchewan
- Demonstrated ability to deal with cultural issues sensitively
- Demonstrated ability to initiate and maintain collaborative working relationships
- Demonstrated ability to communicate effectively both verbally and in writing
- Strong understanding of existing health systems and policies at the regional, provincial and federal levels
**Additional requirements**:
- Valid Saskatchewan driver’s license and access reliable transportation
- Must undergo a Criminal Record Check with the Vulnerable Sector

**Job Types**: Part-time, Fixed term contract
Pay: $32,500.00 per year
