**Job Summary**:
**Key Responsibilities**:
- Conduct comprehensive intake assessments of youth referred to YOUCAN programs, evaluating their needs, strengths, and goals.
- Coordinate referrals to appropriate YOUCAN programs and services, collaborating with internal and external stakeholders.
- Provide ongoing case management support to youth enrolled in YOUCAN programs, utilizing a flexible and team-based approach, monitoring progress, and addressing barriers to success.
- Maintain accurate records of intake assessments, service referrals, and client progress, ensuring compliance with organizational standards.
- Schedule and collaborate with co-workers for intake interviews with prospective youth participants, ensuring a welcoming environment.
- Prepare all necessary paperwork for intake, including consent forms and program agreements, in accordance with organizational policies.
- Actively promote YOUCAN's Road to Work and VERTO programs in schools, youth programs, and the community.
- Collaborate with the Employment Programs Manager and other team members to enhance program effectiveness and client outcomes.
- Advocate for youth within the organization and the broader community, ensuring their voices are heard and needs addressed.
- Other duties as assigned.
**Qualifications**:
- Bachelor’s degree in social work, psychology, counseling, or a related field preferred.
- Previous experience working with youth in a social services or community-based organization.
- Strong assessment, case management, and communication skills.
- Knowledge of community resources and social service systems.
- Empathy, patience, and a genuine commitment to supporting youth.
- Ability to work collaboratively in a fast-paced, dynamic environment.
- Commitment to upholding the values and mission of YOUCAN Youth Services.

Pay: $54,000.00-$60,000.00 per year
