**Summary of Function**
Under the supervision of the Care Team Supervisor, the Client Care Worker provides personal care, medication assistance and support to clients in specialized units/programs. They operate within the framework of the policies, procedures, standards and quality & risk management as established by Carefor and the management of the MacKay Centre. The Client Care Worker collaborates with other members of the care team in meeting the needs of individuals to attain an optimal level of independence and well-being and are responsible to work within their scope of practice, performing care in accordance with their training and competence.
**Qualifications**
- Secondary School education, preferred
- Personal Support Worker, Developmental Service Worker or Social Service Worker, preferred or an acceptable combination of training and experience
- Current First Aid & CPR Certification, required
- Gentle Persuasive Approach Training, Crisis Prevention Intervention Training, or similar training, preferred
**Experience**
- 1+ years’ experience working with seniors or adults of various groups with dementia, Alzheimer’s, and mental health illnesses, esp. developmental delays.
- Experience providing personal care, required
- Experience working in a supportive group home, retirement residence, nursing home or similar environment
- Experience in handling confidential information
- Experience with medication assistance
**Skills**
- Ability to work a variety of shifts, in a 24/7 operation - days, evenings, nights & weekends.
- Ability to observe, evaluate and assess situations and use policies and procedures as guidelines
- Effective problem solving and decision making skills
- Effective communication skills (verbal, written)
- Ability to work effectively in a multidisciplinary team and independently with mínimal supervision in a busy, fast paced environment
- Dependable, trustworthy, compassionate with good work habits
- Ability to understand directions and delegations
- Ability and availability to work shifts in a 24 hour operation
- Ability to bend, sit, stand, kneel for short periods of time
**Responsibilities**
- Assists or provides care according to the Care Plan and in compliance with a CCW’s scope of practice. And reflects the needs of the client.
- Provides personal care which includes assistance with all aspects of activities of daily living including but not limited to: bathing, dressing and grooming, mouth and skin care, lifts and transfers, light housekeeping including laundry and tidying, assisting with medications or other delegated tasks, exercises, assists with climbing or descending stairs, walking, etc.
- Escorts residents to and from dining room, assists with feeding and serving the residents.
- Observes client for changes in behaviour, signs or symptoms of decreasing health status and gives verbal/written report to RPN or other team members.
- Evaluates the individual's need for change in their personal support care plan and makes recommendations to RPN or designate.
- Recognizes the social and emotional need of the clients and deals with them effectively.
- Contributes to the development of the personal support care plan by reporting, to the RPN or designate, all observations or information obtained from the resident and/or family regarding needs and the effect of the care he/she provided.
- Plans and organizes each day’s activities, setting priorities for personal support and other activities, to effectively utilize time, energy, supplies and equipment.
- Assists with Meal set-up, planning and preparing nutritious meals including special diets.
- Performs light housekeeping/cleaning duties. Including laundry.
- Makes appointments for clients and accompanies them on outings when necessary.
- Encourages and promotes client independence and treats clients with dignity and respect.
- Completes documentation according to established procedures to maintain the communication to other health team members and provide an accurate account of service provided
- Informs the manager or designate if they require additional training with tasks.
- Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Care_f_or policies and procedures
- Performs other duties/tasks as assigned by manager
