Job Summary
Through the lens of the Social Determinants of Health, the Community Health Worker (CHW) coordinates, delivers, and evaluates health promotion initiatives aligned with the Community Health Team’s Health Promotion portfolio. The CHW helps to build inclusive and healthy communities by increasing social cohesion in priority neighbourhoods. The CHW focuses on resident capacity building to foster positive relationships with community partners and help residents to support community development initiatives. This position requires excellent interpersonal, facilitation, and conflict resolution skills.
**Program Description**:
As part of a larger Health Promotion team, the Community Health Worker will have primary responsibility for two key programs: the Multicultural Seniors Program and the Bayshore Program. The Multicultural Seniors Program supports seniors across Ottawa who are at risk of social isolation and face multiple barriers to engagement. It includes four language-specific groups—English, Chinese, Russian, and Arabic. The Community Health Worker will plan and deliver culturally responsive activities, workshops, and outings tailored to each group’s needs, while aligning with overall program objectives.
The Bayshore Program serves residents of the Bayshore neighbourhood and is guided by the Community Development Framework. The Community Health Worker will lead the coordination of large community events, workshops, and resident-led committee activities. This includes working closely with the Community Worker, resident leaders, and local partners to support inclusive, community-driven programming and strengthen resident engagement.
Job-Specific Responsibilities
Program Development and Outreach
- Creates a supportive environment to address barriers individuals and groups may be experiencing
- Develop, deliver and facilitate programs
- Develops and facilitates community-based leadership, self-advocacy and empowerment workshops for priority groups that strengthen positive health outcomes, civic engagement, and build social cohesion
- Promotes the program and the recruitment of residents to participate in workshops and related community development activities
- Ensure seamless coordination of services and support throughout the client’s journey through referrals to other internal or external social services
- In partnership with resident leaders, facilitates a diversity of community development activities in priority neighbourhoods to create inclusive community spaces for residents
- Supports community engagement and resident-capacity building with the goal of increasing sense of belonging and inclusion, fostering supportive networks, and building trust

Collaboration and External Partners (Outreach and Crisis Response)
- Conduct outreach and establish constructive relationships with a broad range of external services, such as Community Houses, immigrant serving agencies, and addiction and mental health services, and use these relationships to facilitate clients’ access.
- Liaises with relevant community partners and community members building connections amongst them such as Tenant Associations, law enforcement, landlords, mental health and substance use health partners, and community members
- Collaborate with internal partners to support clients in achieving their goals and participate in case conferences where appropriate.
- Works with communities to identify areas of potential conflict; assists in implementing conflict resolution methods and supports the coordination of crisis responses

Monitoring and Evaluation
- Actively monitors, evaluates, tracks referrals, and contributes to the development and completion of program reports
- Participates in the continuous coordination of needs assessments with other program staff and partners to identify assets and emerging needs within the community
- Maintains accurate and confidential records for program participation, tracks referrals, and completes incident reports.
- In collaboration with the Health Promoter, monitors the program budget
